Standard care includes early mobilization with sitting out of bed three times daily, allied health professional involvement (physiotherapist, occupational therapist, and dietitian), and routine orientation assessment.\n\nParticipants found to be disoriented are screened for delirium using the validated 4AT assessment tool administered by nursing staff. Those identified as at-risk or experiencing delirium receive a comprehensive delirium care bundle including environmental modifications, medication review, and enhanced monitoring. Standard reorientation interventions include whiteboard orientation displays and purple wrist tags for participants at risk of cognitive impairment to ensure enhanced monitoring and fall prevention.\n\nNo study-specific interventions are provided to this group beyond routine hospital care protocols.",null,[20],{"type":21,"name":9,"description":11,"armGroupLabels":22,"otherNames":18},"BEHAVIORAL",[9],[24],{"name":25,"role":26,"phone":27,"phoneExt":18,"email":28},"Elaine Goh, RN, BSN","CONTACT","+65 82288624","elaine.goh.sh@skh.com.sg",[30],{"facility":5,"status":31,"city":32,"state":18,"zip":33,"country":32,"countryCode":34,"cosmosGeoPoint":35,"geoPoint":40,"contacts":41},"RECRUITING","Singapore","544886","SG",{"type":36,"coordinates":37},"Point",[38,39],103.85007,1.28967,{"lat":39,"lon":38},[42,43,45,48,50,52],{"name":25,"role":26,"phone":27,"phoneExt":18,"email":28},{"name":25,"role":44,"phone":18,"phoneExt":18,"email":18},"PRINCIPAL_INVESTIGATOR",{"name":46,"role":47,"phone":18,"phoneExt":18,"email":18},"Sandy Ang, RN, BSN","SUB_INVESTIGATOR",{"name":49,"role":47,"phone":18,"phoneExt":18,"email":18},"Hui Yu Pang, RN, BSN",{"name":51,"role":47,"phone":18,"phoneExt":18,"email":18},"Di Zhang, RN, DNP",{"name":53,"role":47,"phone":18,"phoneExt":18,"email":18},"Yoke Hui Wong, RN, DNP",{"type":55,"investigatorFullName":18,"investigatorTitle":18,"investigatorAffiliation":18,"oldNameTitle":18,"oldOrganization":18},"SPONSOR","100623430","audio-recorded-messages-delivered-via-ipad-to-prevent-delirium-in-hip-fracture-patients-100623430",false,"NCT07396532","Audio-Recorded Messages Delivered Via iPad to Prevent Delirium in Hip Fracture Patients","Nurse-Led Audio-Recorded Reorientation to Reduce Delirium in Elderly Hip Fractures: A Pilot Randomized Control Trial","Inclusion Criteria:\n\n* Older adults aged 60 years and above\n* Undergoing hip fracture repair surgery\n* No history of cognitive impairment as assessed by Abbreviated Mental Test (AMT)\n* Able to understand to any of the basic languages of Singapore (English, Malay, Mandarin, Tamil)\n\nExclusion Criteria:\n\n* Patients not admitted directly to orthopaedic (study) wards post-operation (such as those requiring post-operative high-dependency unit care)\n* Patients managed conservatively without surgical intervention.\n* Patients admitted for periprosthetic hip fracture repairs\n* Patients who are deaf and mute",true,"ALL","60 Years",{"count":67,"type":68},30,"ESTIMATED","INTERVENTIONAL",[71],"NA","The goal of this pilot clinical trial is to learn if personalized audio-recorded reorientation messages can be successfully implemented in general hospital wards for elderly patients after hip fracture surgery. It will also explore whether this intervention might help prevent confusion (delirium) after surgery. The main questions it aims to answer are:\n\n* Can the intervention be successfully delivered by nursing staff in a busy orthopedic ward?\n* How many eligible patients can be recruited and retained in the study?\n* Do patients tolerate listening to the personalized audio messages without problems?\n* Does listening to personalized audio messages show any early signs of reducing confusion after surgery?\n\nResearchers will compare patients who receive personalized audio messages to those who receive usual care to see if the intervention is feasible to implement and whether it shows promise for preventing delirium.\n\nParticipants will:\n\n* Listen to 2-minute personalized audio-recorded reorientation messages played through an iPad and headphones three times daily for the first 3 days after surgery\n* Have their orientation and mental status checked daily for three times by nursing staff\n* Continue with all their regular medical care and treatments\n* Be monitored until discharge from the study wards.",[74,75,76],"Delirium - Postoperative","Delirium and Post-operative Cognitive Dysfunction (POCD)","Delirium, Postoperative",[78,79,80,81],"delirium","nurse-led","reality reorientation","AI","2026-02-02",{"date":84,"type":85},"2026-02-09","ACTUAL",{"date":87,"type":85},"2025-09-29",{"date":89,"type":68},"2026-07",{"name":5,"class":6},1]
