By clearing the CO2 that normally remains in the upper airway after exhalation (anatomical dead space), TF can dramatically reduce anatomical dead space without the need to increase the delivered VT, making it a safe strategy in terms of lung protection. This reduction in dead space reduces the ventilatory demands of the patients, reducing respiratory drive.",[13],"Device: Throughflow titration phase",[15],{"type":16,"name":17,"description":18,"armGroupLabels":19,"otherNames":20},"DEVICE","Throughflow titration phase","Ventilation will be applied in the TF mode with TF set to 0 LPM for 10 minutes. Patients will receive assist through the NAVA line of the tri-piece (NAVA set to similar settings as Servo period, \\~60-80 LPM flow). Ventilation with Throughflow will be started at a TF flow of 5 LPM, and the NAVA flow will be reduced by 5 LPM. After 10 minutes measurements will be collected. If Edi is greater than or equal to 4 µV, TF flow will be increased to 10 LPM and NAVA flow will be adjusted to keep total flow constant. Measurements will be collected again after 10 minutes. TF flow will be increased in steps of 5 LPM and measurements collected every 10 minutes until Edi is below 3 µV or TF flow reaches the total flow observed during the NAVA period.\n\nAfter either the Edi target has been met or TF flow has reached the total flow, ventilation with TF will be reduced in steps of 5 LPM every 10 minutes (the reverse of the above), while NAVA flow is accordingly adjusted to keep total flow constant.",[9],null,[22,25],{"name":23,"affiliation":5,"role":24},"Ewan Goligher, MD, PhD","PRINCIPAL_INVESTIGATOR",{"name":26,"affiliation":5,"role":24},"Lorenzo Del Sorbo",[28],{"name":29,"role":30,"phone":31,"phoneExt":32,"email":33},"Rongyu ( Cindy) Jin","CONTACT","4163404800","7613","rongyu.jin@uhn.ca",[35],{"facility":36,"status":37,"city":38,"state":39,"zip":40,"country":41,"countryCode":42,"cosmosGeoPoint":43,"geoPoint":48,"contacts":49},"University Health Network","RECRUITING","Toronto","Ontario","M5G 2N2","Canada","CA",{"type":44,"coordinates":45},"Point",[46,47],-79.39864,43.70643,{"lat":47,"lon":46},[50],{"name":51,"role":30,"phone":20,"phoneExt":20,"email":33},"Cindy Jin",{"type":53,"investigatorFullName":20,"investigatorTitle":20,"investigatorAffiliation":20,"oldNameTitle":20,"oldOrganization":20},"SPONSOR",[55],{"name":56,"class":6},"Unity Health Toronto","100488647","feasibility-of-reducing-respiratory-drive-using-the-through-flow-system-100488647",false,"NCT05642832","Feasibility of Reducing Respiratory Drive Using the Through-flow System","Feasibility of Reducing Respiratory Drive in Patients with Acute Hypoxemic Respiratory Failure Using the Through-flow System","Inclusion Criteria:\n\n* PaO2\u002FFiO2 less than or equal to 300 at time of screening\n* Oral endotracheal intubation with ETT 7.5 or 8.0 and on invasive mechanical ventilation\n* Bilateral airspace opacities on chest radiograph or chest CT scan\n\nExclusion Criteria:\n\n* Contraindication to esophageal catheterization (upper gastrointestinal tract surgery within preceding 6 weeks, bleeding esophageal\u002Fgastric varices)\n* Intubation for traumatic brain injury or stroke\n* Intracranial hypertension (suspected or diagnosed by medical team)\n* Anticipated liberation from mechanical ventilation within 24 hours","ALL","18 Years",{"count":67,"type":68},15,"ESTIMATED","INTERVENTIONAL",[71],"NA","Mechanical ventilation can lead to diaphragm and lung injury.
