Participants will run the Café with supervision from a researcher, serving customers prepackaged food and drinks per our protocol, along with guidance and direction from a therapist, once a week. Participants will be responsible for interacting with customers and arranging supplies. On a weekly basis, participants will be asked how they feel (for example tired, stressed, overwhelmed, etc) before and after their session and to rate how well they performed in the café.",[9],[25],{"name":26,"affiliation":5,"role":27},"James Sulzer, PhD","PRINCIPAL_INVESTIGATOR",[29,34],{"name":30,"role":31,"phone":32,"phoneExt":18,"email":33},"Krissy Hansen, PT, ATP","CONTACT","216-957-3584","khansen1@metrohealth.org",{"name":35,"role":31,"phone":18,"phoneExt":18,"email":36},"Tom Nown","tnown@metrohealth.org",[38],{"facility":5,"status":39,"city":40,"state":41,"zip":42,"country":43,"countryCode":44,"cosmosGeoPoint":45,"geoPoint":50,"contacts":51},"RECRUITING","Cleveland","Ohio","44109","United States","US",{"type":46,"coordinates":47},"Point",[48,49],-81.69541,41.4995,{"lat":49,"lon":48},[52,56,57],{"name":53,"role":31,"phone":54,"phoneExt":18,"email":55},"KIm Walsh, OT","216-957-3512","kwalsh3@metrohealth.org",{"name":30,"role":31,"phone":32,"phoneExt":18,"email":33},{"name":58,"role":27,"phone":18,"phoneExt":18,"email":18},"Megan Moynahan",{"type":27,"investigatorFullName":60,"investigatorTitle":61,"investigatorAffiliation":5,"oldNameTitle":18,"oldOrganization":18},"James Sulzer","Principal Investigator","100572496","multimodal-enriched-environment-for-rehabilitation-in-chronic-traumatic-brain-injury-100572496",false,"NCT06734026","Multimodal, Enriched Environment for Rehabilitation in Chronic Traumatic Brain Injury","Multimodal, Enriched Environment for Rehabilitation","Inclusion Criteria:\n\n* moderate to severe neurological diagnosis as diagnosed by a clinician,\n* ability to comprehend multi step instructions,\n* ability to independently balance with weight support,\n* physical deficits resulting in need for additional functional goals,\n* ability to endure 2 hours of weight-supported standing,\n* ability to provide informed consent,\n* basic communication ability,\n* currently discharged from therapy,\n* \\>3 months post-injury,\n* \\>18 years of age at time of enrollment\n\nExclusion Criteria:\n\n* Adults unable to consent\n* Individuals who are not yet adults (infants, children, teenagers)\n* Non-English speaking\n* Pregnant women\n* Prisoners\n* Medical instability\n* Other neurological conditions\n* Severe problems with maintaining follow-up expected (such as, but not limited to history of substance abuse, homelessness\u002Fincarceration, dementia and psychotic disorders)","ALL","18 Years",{"count":72,"type":73},20,"ESTIMATED","INTERVENTIONAL",[76],"NA","The goal of this study is to learn if a enhanced environment (Metro Café) with multi model approach (an approach that combines several therapies) in chronic traumatic brain injury improves outcomes of recovery better than traditional therapy. The main questions it aims to answer are:\n\n* Determine the amount of therapy dosage (speech, cognition, arm and leg movements) obtained during the Metro Café therapy session.\n* Evaluate the connection between environmental enrichment dosage (the Metro Café) and clinical outcome scores.\n\nResearchers will compare Metro Café Treatment to those who do not receive this treatment to see if better improvements in function after a traumatic brain injury are seen in the Metro Café Treatment Participants will train in the Metro Cafe during the 2-month gap between assessments, for a total of 18 training sessions, each up to 2 hours, 3 times per week.
