Patients aged ≥18 years\n2. Patients with suspected coronary artery disease (CAD) (e.g., chest pain syndrome or equivalent symptoms) who are evaluated with coronary computed tomography angiography (CCTA)\n3. Coronary computed tomography angiography (CCTA) showing:\n\n   Plaque with at least moderate stenosis in one or more major epicardial coronary arteries; and\n\n   At least two high-risk plaque (HRP) feature at the site of stenotic lesions, defined as any of the following:\n   * Low-attenuation plaque (LAP) (\\\u003C70 Hounsfield units)\n   * Positive remodeling (PR) (remodeling index \\>1.2)\n   * Napkin-ring sign (NRS)\n   * Spotty calcification (SC) (\\\u003C3 mm in length)\n4. Willing and able to provide written informed consent\n\nExclusion Criteria:\n\n1. Acute coronary syndrome (ACS) requiring urgent or emergent invasive evaluation\n2. Hemodynamically unstable conditions\n3. Significant left main coronary artery disease (≥50% diameter stenosis)\n4. Coronary anatomy unsuitable for either percutaneous coronary intervention (PCI) or coronary artery bypass grafting (CABG)\n5. Left ventricular ejection fraction (LVEF) \\\u003C35%\n\n   → Left ventricular ejection fraction \\\u003C35%\n6.
