The posterior peritoneum is opened if considered technically feasible and clinically appropriate. The cardinal-uterosacral ligament complex is clamped, transected, and ligated. The ligament complex is sutured to the proximal part of the anterior cervix. The cervix is amputated using diathermy, and the patency of the cervical canal is ensured. The vaginal epithelium is sutured circumferentially around the remaining cervical stump. Both the anterior and posterior peritoneum are opened. The cardinal-uterosacral ligament complex is clamped, transected, and ligated. The uterus is removed in multiple steps by clamps and sutures. The adnexa are checked for abnormalities. Apical suspension is performed using a 0-polyglactin (braided, delayed-absorbable) suture. The suture incorporates the ligament complex on both sides and passes twice through the peritoneum and the full thickness of the posterior vaginal wall, exiting into the posterior fornix and returning through the same tissue layers. The suture is tightened to obliterate the cul-de-sac. In case of a large enterocele, an additional transverse suture may be placed proximally. The ligament pedicles are tied together in the midline. The vaginal wall is closed. Concomitant anterior and posterior colporrhaphy as well as perineorrhaphy will be performed when indicated.",[15],[33,38],{"name":34,"role":35,"phone":36,"phoneExt":11,"email":37},"Päivi K Karjalainen, MD, PhD","CONTACT","+358142695512","paivi.karjalainen@hyvaks.fi",{"name":39,"role":35,"phone":40,"phoneExt":11,"email":41},"Anniina Karioja, MD","+358331165005","anniina.karioja@pirha.fi",[43,63,74,87,100,112],{"facility":44,"status":45,"city":46,"state":11,"zip":11,"country":47,"countryCode":48,"cosmosGeoPoint":49,"geoPoint":54,"contacts":55},"Helsinki University Hospital","RECRUITING","Helsinki","Finland","FI",{"type":50,"coordinates":51},"Point",[52,53],24.93545,60.16952,{"lat":53,"lon":52},[56,60],{"name":57,"role":35,"phone":58,"phoneExt":11,"email":59},"Tomi Mikkola, Professor","+35894711","tomi.mikkola@hus.fi",{"name":61,"role":35,"phone":11,"phoneExt":11,"email":62},"Camilla Isaksson, MD, PhD","camilla.isaksson@hus.fi",{"facility":64,"status":45,"city":65,"state":11,"zip":11,"country":47,"countryCode":48,"cosmosGeoPoint":66,"geoPoint":70,"contacts":71},"Hospital Nova, The Wellbeing Services County of Central Finland","Jyväskylä",{"type":50,"coordinates":67},[68,69],25.72088,62.24147,{"lat":69,"lon":68},[72],{"name":34,"role":35,"phone":73,"phoneExt":11,"email":37},"+35814 2695512",{"facility":75,"status":45,"city":76,"state":11,"zip":11,"country":47,"countryCode":48,"cosmosGeoPoint":77,"geoPoint":81,"contacts":82},"Kuopio University Hospital","Kuopio",{"type":50,"coordinates":78},[79,80],27.67703,62.89238,{"lat":80,"lon":79},[83],{"name":84,"role":35,"phone":85,"phoneExt":11,"email":86},"Kaisa Raatikainen, Docent","+358 17 173311","kaisa.e.raatikainen@pshyvinvointialue.fi",{"facility":88,"status":45,"city":89,"state":11,"zip":11,"country":47,"countryCode":48,"cosmosGeoPoint":90,"geoPoint":94,"contacts":95},"Oulu University Hospital","Oulu",{"type":50,"coordinates":91},[92,93],25.46816,65.01236,{"lat":93,"lon":92},[96],{"name":97,"role":35,"phone":98,"phoneExt":11,"email":99},"Henna-Riikka Rossi, MD, PhD","+358 50 579 4230","henna.rossi@pohde.fi",{"facility":5,"status":45,"city":101,"state":11,"zip":11,"country":47,"countryCode":48,"cosmosGeoPoint":102,"geoPoint":106,"contacts":107},"Tampere",{"type":50,"coordinates":103},[104,105],23.78712,61.49911,{"lat":105,"lon":104},[108],{"name":109,"role":35,"phone":110,"phoneExt":11,"email":111},"Kirsi Kuismanen, MD, PhD","+358 44 4739961","kirsi.kuismanen@pirha.fi",{"facility":113,"status":45,"city":114,"state":11,"zip":11,"country":47,"countryCode":48,"cosmosGeoPoint":115,"geoPoint":119,"contacts":120},"Turku University Hospital","Turku",{"type":50,"coordinates":116},[117,118],22.26869,60.45148,{"lat":118,"lon":117},[121],{"name":122,"role":35,"phone":123,"phoneExt":11,"email":124},"Kaisa Kurkijärvi, MD, PhD","+358 50 459 0301","kaisa.kurkijarvi@varha.fi",{"type":126,"investigatorFullName":11,"investigatorTitle":11,"investigatorAffiliation":11,"oldNameTitle":11,"oldOrganization":11},"SPONSOR",[128,130,132,133,134],{"name":129,"class":6},"Helsinki University Central Hospital",{"name":113,"class":131},"OTHER_GOV",{"name":75,"class":6},{"name":88,"class":6},{"name":135,"class":6},"Hospital Nova, Central Finland","100636535","the-finnish-uterine-prolapse-surgery-trial-the-fin-up-trial-100636535",false,"NCT07566949","The Finnish Uterine Prolapse Surgery Trial (The Fin-UP Trial)","Manchester Procedure Versus Vaginal Hysterectomy for Women With Uterine Prolapse: A Multi-Center Randomized Controlled Study (The Fin-UP Trial)","Fin-UP","Inclusion Criteria:\n\n* Fit for elective vaginal surgery according to standard preoperative assessment\n* Women aged over 18 years\n* No previous POP surgery\n* Symptomatic POP stage 2 or higher, with uterine descent of at least stage 2 (POP-Q point C ≥ minus 1) and with POP-Q point D ≤ minus 1.
