Age 18 or above\n2. Received a kidney transplant within the past year with functioning kidney (eGFR \\> 30 mL\u002Fmin\n3. Person with Type 2 Diabetes and on insulin\n4. Access to home wi-fi connection\n\nExclusion Criteria:\n\n1. Person with Type 1 Diabetes\n2. Patients taking hydroxyurea\n3.
