Moderate to severe, predominantly medial chronic knee pain\n2. Age adult \\>18 years old\n3. WOMAC Score of 30 or higher\n4. NRS\\>6 with ambulation\n5. Radiological confirmation of knee osteoarthritis (Kellgren-Lawrence Scale: 2-4)\n6. Participants in whom the measured skin to bone depth is 2-6 cm using imaging ultrasound and ultrasound gel pad in the upper medial and lower medial joint capsule targets.\n\nExclusion Criteria:\n\n1. Individuals meeting any of the following criteria are not eligible for participation in this study:\n2. Individuals after any surgery associated with altered integrity of bone structure\n3. Individuals with severe pain in the second knee\n4. Individuals unable to understand and complete the research questionnaires in the official language\n5. Individuals presenting with any severe medical condition preventing the participant from safely and effectively being treated in the study or reporting study outcomes, per PI decision\n6. Individuals with extensive scarring of the skin and tissue overlying the treatment area\n7. Individuals enrolled in or planned to be enrolled in another clinical trial during the duration of this research project\n8. Individuals with active inflammatory (e.g., rheumatoid) arthritis\n9. Individuals with concomitant mental health disorders, excluding compensated mood disorders\n10. Individuals with altered perception and processing of pain: Average NRS \\> 9, PCS\\>30, SSS-8 \\> 11, CSI\\>40 \\[46\\], \\[47\\], \\[48\\]\n11. Individuals with a first-degree family member already enrolled in this study\n12. Individuals who are scheduled for any interventional\u002Fsurgical procedure within 3 months from the screening date, excluding trivial surgeries (e.g., cataract, skin tumors)\n13. Inability to attend follow-up visits and physiotherapy due to travel distance or other restrictions\n14. Symptomatic multifocal nociplastic chronic pain (e.g., fibromyalgia, generalized osteoarthritis) or any other chronic pain condition that could influence symptoms.\n15. The sound waves are tightly focused on a specific area of the nerve. The sound waves cause a break in the nerve which carries pain signals. Sound waves transmit through the skin so there is no need for needles. The procedure takes approximately one hour. The device has been approved in Health Canada and Europe for the treatment of lower back pain. Men and women aged 18 and older\n2. BPI \\\u003C 40\n3. Patients who are able and willing to give consent and able to attend all study visits.\n4. Patients who are able to communicate with the treating physician.\n5. Patients must have chronic lower back pain (LBP) attributed to degenerative sacroiliitis diagnosed by a pain physician using the following guidelines:\n6. At least 1 positive sacroiliac joint provocation test (Gaeslen's test, FABER (Patrick's test), and pelvic rock)\n7. Back pain predominantly below L5\n8. 50% relief following either sacroiliac joint (SI) joint injection and\u002For lateral branch blocks corresponding to the expected duration of the medication utilized. (i.e SI joint injection-expected pain relief duration of steroid, lateral branch blocks-expected pain relief duration of local anesthetic utilized).\n9. All other possible sources of low back pain have been ruled out as the primary pain generator, including but not limited to: the intervertebral discs, bone fracture, the zygapophyseal joints, the hip joint, symptomatic spondylolisthesis, tumor, and other regional soft tissue structures (this is done by physical exam, medical history, and MRI\u002FCT\u002FX-ray as required)\n10. Patients with NRS (0-10 scale) LBP average score ≥ 4\n11. Patients with chronic LBP for at least 12 months.\n12. Patients with Oswestry Disability Questionnaire (ODQ) for LBP score \\> 40%\n\nExclusion Criteria:\n\n1. Patients on dialysis\n2. Patients with evidence of lumbosacral radiculopathy on MRI, CT or physical exam findings, including radicular leg pain (symptomatic moderate or severe foraminal or central canal stenosis).\n3. Spinal pathology that may impede recovery such as spina bifida occulta, grade II or higher.\n4. Spondylolisthesis at L5\u002FS1, or scoliosis\n5. Patients with radiofrequency ablation (RF\u002FRFA) treatment for sacroiliitis LBP\n6. Patients with bilateral sacroiliitis LBP\n7. Patients with previous low back surgery\n8. Patients who are pregnant\n9. Patients with existing malignancy\n10. Patients with allergies to relevant anesthetics\n11. Patients with motor deficit or any other indication for surgical intervention\n12. Patients with contraindications for MRI\n13. Patients with an acute medical condition (e.g., pneumonia, sepsis, systemic infection, uncontrolled immunosuppression-AIDS, cancer, DM) that is expected to hinder them from completing this study.\n14.
