The goal is to determine which intervention program is more effective in treating balance problems after a concussion. Untreated OSA in children has been associated with daytime sleepiness, cognitive or behavioral problems, and cardiovascular complications, all which are common in children with DS. Adenotonsillectomy (AT) is the first line treatment for OSA in children, however, most large studies of AT outcomes have excluded children with DS. Available evidence demonstrates that AT is far less effective in children with DS than in the general pediatric population, with 48 to 95% of children with DS having persistent OSA after AT. Medical treatments such as positive airway pressure (PAP) therapy are frequently inadequate or poorly tolerated in this population, so many children with DS and OSA remain untreated. Drug-induced sleep endoscopy (DISE) enables direct observation of the sites and patterns of obstruction during sedated sleep using a flexible endoscope passed through the nose into the pharynx. DISE was developed to guide surgical decisions in adult OSA, and in recent years has also been used to design personalized surgical interventions in children. Using this DISE Rating Scale, the investigators have demonstrated that children with DS are more prone to tongue base and supraglottic obstruction than non-DS children, suggesting the need for more personalized surgical treatments that are tailored to the common sources of obstruction in this population. Several small case series demonstrate that DISE-directed surgery can be effective in treating OSA in children with DS. Staying on medication for opioid use disorder (MOUD) can greatly reduce this risk, but many people stop treatment during this transition. This study is testing whether a program that offers rewards, called contingency management (CM), can help people stay on MOUD after they return to the community.\n\nIn this study, people who are taking MOUD in jail can choose to participate. While still incarcerated, they complete one visit where they may receive a small incentive, such as commissary credit or snacks, for enrolling in the CM program. After they leave jail, participants can earn gift cards when they attend their MOUD appointments and counseling sessions.\n\nParticipants may take part in the study for up to three months after release. During this time, they can earn up to $600 in incentives based on their treatment engagement. Researchers will also ask participants to complete a baseline survey and a survey at 3 months. We will also review medical records to confirm whether they continued their MOUD.\n\nThe researchers want to learn whether this adapted contingency management program is feasible, acceptable to participants, and helpful for supporting treatment engagement after release from jail. Diagnosis of type 1 diabetes mellitus for at least 1 year.\n2. Age 18 years and older.\n3. Able to read, write and understand spoken English\n4. HbA1c or GMI 7.5- 11.5% at screening. GMI (%) = 3.31 + 0.02392 × \\[mean glucose in mg\u002FdL\\] based on prior 30 days of CGM data with at least 70% time CGM active\\[6\\]\n5. Use of multiple daily insulin injections (MDI) for at least 30 days at the time of screening visit.\n6. Willing to use the approved study insulin and devices while on study - Humalog and Basaglar Tempo pens, Dexcom G6 CGM, study iPhone, Apple watch\n7. Willingness and ability to independently follow all study procedures, including attending all clinic and DSMES and behavioral health visits.\n8. Total daily insulin requirements of less than 200 units\n\nExclusion Criteria:\n\n1.
