Increased Risk of Malnutrition","Inclusion Criteria:\n\n* Informed Consent as documented by signature\n* Male or female 70 years of age or older\n* Hospitalized at site\n* Nutrition Risk Screening 2002 (NRS-2002) ≥ 3\n* eGFR ≥ 30 mL\u002Fmin\u002F1.73 m2 (EPI-CKD) based on medical history (no older than 6 months)\n* Ability to eat orally\n\nExclusion Criteria:\n\n* Patients with acute cardiovascular event ≤ 2 days\n* Patients with terminal conditions\n* Patients diagnosed with pancreatic exocrine insufficiency (EPI) treated with pancreatic enzyme replacement therapy (PERT)\n* Functional Outcomes and Recovery Trial: The EFFORT II Project","EFFORT II","Inclusion Criteria:\n\n* Informed Consent as documented by signature\n* Adult (age ≥18 years), medical patients\n* Nutritional risk screening using the Nutritional Risk Screening (NRS): total score ≥3 points consisting of ≥1 points for impairment of the nutritional status \\[weight loss \\>5% in 3 month or food intake of 50-75% in the last week before hospital admission\\] plus ≥1 for the severity of the disease (i.e., cancer, chronic kidney disease, chronic heart failure, COPD) and other chronic diseases according to the definition of the \"National Center for Chronic Disease Prevention and Health Promotion\": Chronic diseases are defined broadly as conditions that last 1 year or more and require
