The main questions it aims to answer are:\n\nDoes AI-assisted rehabilitation among frail adults improve patients' physical outcomes? Is there a particular subgroup of frail subjects that will benefit most from AI-assisted rehabilitation?\n\nResearchers will compare AI-assisted rehabilitation to standard practice (advice and QR link for rehabilitation videos to be done at home) to see if AI-assisted rehabilitation improves clinical outcomes compared to standard practice.\n\nParticipants will either:\n\nUndergo an AI-assisted rehab with the AI-sensor tool or a standard practice post-discharge from community hospitals for 12 weeks.\n\nUndergo interval assessments of outcomes. Keep a diary of outpatient rehabilitations (if applicable for the subjects).",[27],"Frail",[29,30],"hybrid effectiveness-implementation study","pragmatic","NOT_YET_RECRUITING","2025-09-09",{"date":34,"type":35},"2025-09-16","ACTUAL",{"date":37,"type":21},"2025-12-01",{"date":39,"type":21},"2027-04-30",{"name":41,"class":42},"SingHealth Community Hospitals","OTHER",{"id":44,"slug":45,"hasResults":11,"nctId":46,"briefTitle":47,"officialTitle":47,"acronym":4,"eligibilityCriteria":48,"healthyVolunteers":11,"sex":16,"minAge":49,"maxAge":50,"enrollmentInfo":51,"targetDuration":4,"studyType":22,"phases":53,"briefSummary":54,"conditions":55,"keywords":4,"overallStatus":31,"whyStopped":4,"lastUpdateSubmitDate":58,"lastUpdatePostDateStruct":59,"startDateStruct":61,"completionDateStruct":63,"leadSponsor":65,"locationsCount":4},"100568648","ai-assisted-telerehabilitation-system-atlas-for-post-discharge-continuation-of-rehabilitative-care-100568648","NCT06683963","AI-Assisted TelerehabiLitAtion System (ATLAS) for Post-discharge Continuation of Rehabilitative Care","Inclusion Criteria:\n\n1.
