Older adults (=\\>50 years of age)\n  2. Enrolled in Home Health Care\n  3. Documented to have HF\n  4. Mild to moderate CI on the BIMS or OASIS items\n  5. Able to provide informed consent\n  6. Able to provide self-report data\n* Caregiver Inclusion Criteria\n\n  1. Adults (=\\>18 years of age)\n  2. Spouse\u002Fpartner or child living with or in close proximity to the patient\n  3. Caregiver of a home care patient who is currently receiving or recently received services and who has mild to severe cognitive impairment\n  4. Reporting poor self-care on screening (Health Self-Care Neglect scale score =\\>2)\n  5. Reporting poor mental health on screening (screened with the PROMIS Global Mental Health 4a score \\\u003C12)\n  6. Caregiving or supporting the patient at least 8 hours\u002Fweek\n  7. After collecting baseline data, we will block randomize the caregivers 1:1 to the ViCCY intervention or comparator group, stratifying randomization by caregiver sex (male\u002Ffemale), relationship (spouse\u002Fpartner or child), and race (white\u002Fother)- factors known to influence caregiving burden, perceived stress, and receptivity to the intervention. We will encourage caregivers to use their own devices but provide tablet devices with wireless network access if needed. DECLARE includes a caregiver self-assessment that is reported in the home health patient record, and increased access to social work for cases including dementia caregivers. The main questions we aim to answer are:\n\n* Will most caregivers who are offered the chance to complete an assessment choose to do so?\n* Will clinicians report that the assessment information was useful?\n* Will the rate of social work access increase for cases with patients with dementia who have involved family caregivers?\n\nResearchers will compare DECLARE to usual care to see if taking part in the program increases caregiver self-efficacy and access to social work.\n\nParticipants will:\n\n* Answer a short series of assessment questions and questions about their caregiving experiences at the beginning of the home health episode.\n* Receive a social work visit from a Licensed Social Worker trained in dementia care.\n* Answer a series of follow-up questions about their caregiving experiences at the end of the home health episode.",[64,65],"Alzheimer Disease or Associated Disorder","Dementia","RECRUITING","2026-03-10",{"date":69,"type":42},"2026-03-13",{"date":71,"type":42},"2025-05-05",{"date":73,"type":22},"2026-06-30",{"name":48,"class":49},1,""]
