Progressive tendon loading programs are considered first-line treatment but do not lead to satisfactory outcomes in all patients. Extracorporeal shockwave therapy (ESWT) and platelet-rich plasma (PRP) injections are commonly used interventions for refractory symptoms, yet evidence supporting their combined use is limited particularly in treating Achilles tendon disorders.\n\nThis randomized controlled trial will evaluate the effectiveness of a standardized exercise program alone or in combination with PRP injection, ESWT, or both therapies in active adults with midportion Achilles tendinopathy. Participants will be randomized to one of four treatment groups and followed for six months. The primary outcome will assess changes in Achilles tendon symptoms and function using the Victorian Institute of Sport Assessment-Achilles (VISA-A) score. The goal is to collect detailed information over time to understand which treatments work best for different people, identify risk factors for injury, and improve overall care. Adults over 18, both with and without Achilles tendon conditions, are eligible to join. Participants will attend several assessment visits at the Prince of Wales Hospital over the first year (at the start, 6 weeks, 3, 4, 6, and 12 months) for physical tests, ultrasounds, and questionnaires. After the first year, follow-up continues with annual online questionnaires for up to five years. The main measure is a standard questionnaire about tendon pain and function (VISA-A). The study aims to use this information to guide better, more personalized rehabilitation strategies, improve patient outcomes, and reduce long-term disability. Conservative management of MAT has been suggested as the first line of treatment. Actually, there are several exercise programs with beneficial effects on pain and function among which high load training stands out. However, this training modality is inherently difficult to implement in certain populations and pathological conditions due to their inability to tolerate this mechanical stress. In this sense, low load training with blood flow restriction (BFR-LLT) emerges as an effective option in producing hypertrophic adaptations with low intensities (30% 1RM).
