A detailed medical history and routine preoperative assessment will be conducted.\n\nClinical evaluation will include identification of the internal and external openings, assessment of discharge, and continence status using the Jorge-Wexner incontinence score. MRI fistulogram will be performed preoperatively to evaluate the fistula tract and its relation to the sphincter complex.\n\nPatients will be randomly allocated into two equal groups (1:1 ratio) using a computer-generated sequence:\n\nGroup A: Undergo LIFT procedure Group B: Undergo IFOC procedure Both procedures will be performed as per standard surgical techniques. Postoperatively, patients will start oral fluids after 2 hours and resume a normal diet as tolerated. Discharge is planned on the first postoperative day unless otherwise indicated. Surgical management aims to eradicate the fistulous tract while preserving anal sphincter function and continence.\n\nDespite numerous available surgical techniques, high-quality comparative evidence regarding optimal management remains limited. This prospective observational study aims to evaluate clinical outcomes, functional outcomes, and patient-reported quality of life following surgical treatment of perianal fistulas.\n\nThe study will collect both clinician-reported and patient-reported outcomes over a 12-month follow-up period. Outcomes of interest include fistula healing, recurrence, postoperative complications, continence status, symptom burden, and health-related quality of life. The findings are expected to provide real-world data that may inform clinical decision-making and contribute to improved patient-centered care.",[26,59,60],"Anal Fistula","Perianal Fistula",[62,63,64,65,66,67],"perianal","fistula","anal","PROMs","patient-reported outcomes","quality of life","2026-03-17",{"date":70,"type":36},"2026-03-19",{"date":72,"type":36},"2026-01-01",{"date":74,"type":20},"2030-01-01",{"name":76,"class":43},"National and Kapodistrian University of Athens",{"id":78,"slug":79,"hasResults":11,"nctId":80,"briefTitle":81,"officialTitle":82,"acronym":4,"eligibilityCriteria":83,"healthyVolunteers":11,"sex":16,"minAge":17,"maxAge":4,"enrollmentInfo":84,"targetDuration":4,"studyType":56,"phases":4,"briefSummary":86,"conditions":87,"keywords":89,"overallStatus":93,"whyStopped":4,"lastUpdateSubmitDate":94,"lastUpdatePostDateStruct":95,"startDateStruct":96,"completionDateStruct":98,"leadSponsor":100,"locationsCount":4},"100630020","impact-of-surgical-timing-on-anal-fistula-outcomes-100630020","NCT07482241","Impact of Surgical Timing on Anal Fistula Outcomes","Does the Duration Between Symptom Onset and Surgical Intervention Affect the Outcomes in Patients With Anal Fistula?","Inclusion Criteria:\n\n* Patients aged 18 years or older diagnosed with primary cryptoglandular anal fistula confirmed by clinical examination and preoperative imaging (MRI).\n* Patients who will be undergoing definitive surgical intervention for anal fistula at Assiut University Hospital between 2026 and 2028.\n* Complete medical records available, including documented symptom onset date (defined as first reported perianal pain, swelling, or discharge) and surgery date.\n* Minimum postoperative follow-up of 12 months, with clinical assessment data on healing and recurrence.\n\nExclusion Criteria:\n\n* Fistulas due to Crohn's disease, tuberculosis, malignancy, trauma, radiation, or obstetrical injury.\n* Pregnant women.\n* Recurrent anal fistulas or those with prior surgical intervention for the same fistula.\n* Incomplete records or loss to follow-up before 6 months postoperatively.\n* Patients with comorbidities precluding surgery (e.g., uncontrolled coagulopathy or active systemic infection).",{"count":85,"type":20},90,"This study aims to evaluate if the amount of time a person experiences symptoms of an anal fistula before having surgery affects their surgical outcomes and recovery.\n\nAn anal fistula is a chronic condition that typically requires surgery to heal properly. In routine clinical practice, many patients experience a significant delay before getting surgery due to a variety of reasons, such as under-reporting symptoms, misdiagnosis, or prolonged conservative treatments. Doctors want to understand if this delay in surgical intervention allows ongoing infection and scarring to complicate the surgery and worsen the patient's recovery.\n\nTo investigate this, researchers will observe 90 adult patients who are undergoing definitive surgery for a primary anal fistula. The participants will be divided into three groups based on how long they had symptoms before their operation:\n\n3 months or less Between 3 and 6 months More than 6 months\n\nThe main goal of the study is to compare these groups to see how many patients achieve complete clinical healing and how many experience a recurrence (the fistula returning) within 12 months after the surgery. Additionally, the study will track secondary outcomes, including how long it takes the wound to heal completely, any postoperative complications, changes in bowel control (fecal incontinence), pain resolution, and the patient's overall postoperative quality of life. The findings will help inform both doctors and patients on the optimal timing for anal fistula surgery.",[26,59,88],"Surgery Timing",[59,90,91,92],"Cryptoglandular Anal Fistula","Fistula in Ano","Anorectal Fistula","NOT_YET_RECRUITING","2026-03-16",{"date":70,"type":36},{"date":97,"type":20},"2026-05",{"date":99,"type":20},"2028-06",{"name":101,"class":43},"Assiut University",{"id":103,"slug":104,"hasResults":11,"nctId":105,"briefTitle":106,"officialTitle":106,"acronym":4,"eligibilityCriteria":107,"healthyVolunteers":11,"sex":16,"minAge":17,"maxAge":4,"enrollmentInfo":108,"targetDuration":4,"studyType":56,"phases":4,"briefSummary":110,"conditions":111,"keywords":112,"overallStatus":93,"whyStopped":4,"lastUpdateSubmitDate":115,"lastUpdatePostDateStruct":116,"startDateStruct":118,"completionDateStruct":120,"leadSponsor":122,"locationsCount":4},"100561591","factors-influencing-wound-healing-after-anal-fistula-surgery-100561591","NCT06592157","Factors Influencing Wound Healing After Anal Fistula Surgery","Inclusion Criteria:\n\n* Patients over 18 years old.\n* Have undergone anal fistula surgery with complete surgical records.\n* No significant surgical contraindications, such as:Acute infection or inflammation,Severe cardiopulmonary insufficiency,Coagulation disorders,Immunodeficiency,Local skin lesions.\n* Understand and are willing to participate in the study.Willing to sign the informed consent form.\n\nExclusion Criteria:\n\n* Patients under 18 years of age.\n* Pregnant or breastfeeding women.\n* Individuals with pre-existing severe heart, liver, or kidney diseases.\n* Individuals with active infections or systemic diseases.\n* Individuals who have previously undergone similar surgeries.\n* Individuals with immune dysfunction (such as HIV infection).",{"count":109,"type":20},600,"The objective of this observational study is to analyze the factors influencing wound healing after anal fistula surgery and to assess the effectiveness of interventions in improving overall patient outcomes post-surgery.
