Caregiver must be competent to call care team if a problem is evident to her\u002Fhim. After 24 hours, this caregiver should be available for as-needed spot checks on the patient. This criterion may be waived for highly competent patients at the patient and clinician's discretion.\n* Primary or possible diagnosis of cellulitis, heart failure, complicated urinary tract infection, pneumonia, COPD\u002Fasthma, other infection, chronic kidney disease, malignant pain, diabetes and its complications, gout flare, hypertensive urgency, previously diagnosed atrial fibrillation with rapid ventricular response, anticoagulation needs, or a patient who desires only medical management that requires inpatient admission, as determined by the emergency room team.\n\nExclusion Criteria:\n\n* Undomiciled\n* No working heat (October-April), no working air conditioning if forecast \\> 80°F (June-September), or no running water\n* On methadone requiring daily pickup of medication\n* In police custody\n* Resides in facility that provides on-site medical care (e.g., skilled nursing facility)\n* Domestic violence screen positive\n* Acute delirium, as determined by the Confusion Assessment Method\n* Cannot establish peripheral access in emergency department (or access requires ultrasound guidance)\n* Secondary condition: end-stage renal disease, acute myocardial infarction, acute cerebral vascular accident, acute hemorrhage\n* Primary diagnosis requires multiple or routine administrations of intravenous narcotics for pain control\n* Cannot independently ambulate to bedside commode\n* As deemed by on-call medical doctor, patient likely to require any of the following procedures: computed tomography, magnetic resonance imaging, endoscopic procedure, blood transfusion, cardiac stress test, or surgery\n* High risk for clinical deterioration\n* Home hospital census is full (maximum 5 patients at any time)","ALL","18 Years",{"count":19,"type":20},3000,"ESTIMATED","INTERVENTIONAL",[23],"NA","The investigators propose a home hospital model of care that substitutes for treatment in an acute care hospital. Age ≥ 18 years old;\n2. Adult patients with objectively diagnosed pulmonary thromboembolism (PTE) (with or without concomitant deep vein thrombosis);\n3. Specific populations meeting any of the following criteria: elderly patients (≥75 years old); patients with impaired kidney function (creatinine clearance estimated by the Cockcroft-Gault formula, CrCl \\\u003C 60ml\u002Fmin); patients with active malignancies (under treatment or with unrecovered malignancies);\n4. Patients who have not started anticoagulant therapy before enrollment and are planned for anticoagulant treatment after evaluation by the attending physician;\n5. Ecpected life expectancy longer than 3 months.\n6. Patients who understand and agree to participate in this study, sign the informed consent form, and adhere to regular follow-up visits.\n\nExclusion Criteria:\n\n1. Moderate or severe liver dysfunction (Child-Pugh class B or C);\n2. Spontaneous bleeding tendency, such as coagulation disorders or thrombocytopenia (PLT\\\u003C20×10\\^9\u002FL);\n3. Contraindications to anticoagulant drugs, including allergy to anticoagulants, clinically significant active bleeding, significant risk of major bleeding due to lesions or conditions, significantly abnormal coagulation function, liver disease with clinically relevant bleeding risk, thrombocytopenia, etc., as determined by the attending physician;\n4. High-risk pulmonary embolism requiring thrombolysis;\n5. Patients currently participating in other clinical trials.",{"count":55,"type":20},4700,"OBSERVATIONAL","The goal of this prospective observational study is to explore the influencing factors of the efficacy and safety of anticoagulant therapy for pulmonary thromboembolism in special populations with older age, renal insufficiency or co-existing malignancy, and establish a predictive model to guide clinical practice. The main questions it aims to answer are:\n\n* To analyze the influencing factors of the efficacy and safety of anticoagulant therapy for specific populations with pulmonary thromboembolism (PTE) in the real world (such as the elderly, those with impaired kidney function, and individuals with malignant tumors).\n* Whether we can use machine-learning models to predict bleeding events and VTE recurrence in special populations following anticoagulant therapy in the real world? Participants will receive diagnostic and therapeutic measures for pulmonary thromboembolism in accordance with clinical guidelines, including anticoagulant therapy.
