Presence of chronic suppurative apical periodontitis with an apicomarginal defect and deep periodontal pocket, suggesting endodontic-periodontal communication up to the apex.\n\nNegative response to pulp vitality tests.\n\nExclusion Criteria:\n\nPresence of root fractures or perforations. Patients with compromised systemic health (ASA II or above). Guided tissue regeneration using grafts and collagen membranes may improve outcomes, but the comparative effectiveness of autografts and allografts in these defects is not well established. This randomized clinical trial at PGIDS Rohtak will evaluate 36 patients undergoing endodontic microsurgery, assigning them to either autograft or allograft combined with a bioresorbable collagen membrane. Clinical and CBCT-based radiographic healing will be monitored over 12 months. Patients with no general medical contraindications for oral surgical procedures (ASA-I and ASA-II according to the classification of the American Society of Anesthesiologist's).\n2. Patients aged 18 years and above, presenting with chronic suppurative apical periodontitis in at least one tooth,\n3. Radiographic evidence of apical radiolucency with periodontal pocket depth exceeding 6 mm confined to buccal aspect of the root.\n4. Repeated abscess discharge despite satisfactory root canal treatment. 5.Patients with apicomarginal defects showing a denuded buccal bone plate\n\n6.Failed primary root canal treatment 7.Adequate final restoration with no clinical evidence of coronal leakage.\n\nExclusion Criteria:\n\n1. Clinical or radiographic evidence of root fracture.\n2. Resorptive processes involving more than apical third of the root.\n3. Chronic generalized periodontitis.\n4. Failed previous endodontic surgery or failed root canal re-treatment.\n5.
