While successful catheter ablation has been shown to reverse some aspects of atrial remodeling, the impact of pulsed-field ablation (PFA) - a non-thermal ablation technology that selectively targets cardiomyocytes while sparing surrounding structures - on atrial remodeling in patients with persistent AF undergoing extensive substrate modification remains unclear.\n\nThis is a prospective, single-center, single-arm observational study conducted at Taipei Veterans General Hospital. The study will enroll 30 patients with persistent atrial fibrillation who undergo pulmonary vein isolation (PVI) and extensive substrate modification using the FARAWAVE \u002F FARADRIVE Catheter and FARASTAR System.\n\nThe primary objective is to evaluate left atrial structural remodeling following PFA, including changes in LA size, atrial strain, extent of atrial fibrosis, and atrial hemodynamic function. The secondary objective is to assess AF-free survival between 3 and 12 months after the procedure, and its relationship to atrial remodeling. All participants will be followed for 12 months, with an interim analysis conducted once all subjects complete their 6-month follow-up.",[26,27,28,29],"Persistent Atrial Fibrillation","Atrial Fibrillation (AF)","Atrial Remodeling","Pulsed Field Ablation",[29,26,31,28,32,33,34,35],"Atrial Fibrillation","Substrate Modification","Atrial Fibrosis","Cardiac MRI","Posterior Wall Isolation","NOT_YET_RECRUITING","2026-06-30",{"date":39,"type":40},"2026-07-02","ACTUAL",{"date":42,"type":20},"2026-07-01",{"date":44,"type":20},"2028-12-31",{"name":46,"class":47},"Taipei Veterans General Hospital, Taiwan","OTHER_GOV",1,{"id":50,"slug":51,"hasResults":11,"nctId":52,"briefTitle":53,"officialTitle":53,"acronym":54,"eligibilityCriteria":55,"healthyVolunteers":11,"sex":16,"minAge":17,"maxAge":4,"enrollmentInfo":56,"targetDuration":4,"studyType":58,"phases":4,"briefSummary":59,"conditions":60,"keywords":4,"overallStatus":61,"whyStopped":4,"lastUpdateSubmitDate":62,"lastUpdatePostDateStruct":63,"startDateStruct":65,"completionDateStruct":67,"leadSponsor":69,"locationsCount":48},"100548809","characterization-of-left-atrial-substrate-in-patients-with-ventricular-arrhythmias-100548809","NCT06425822","Characterization of Left Atrial Substrate in Patients With Ventricular Arrhythmias","CLASS-VA","Inclusion Criteria:\n\n* Absence of a prior history of atrial fibrillation or flutter.\n* Patients presenting for ablation of any ventricular tachycardia related to structural heart disease or any ventricular arrhythmia with a CHA2DS2-VASc score ≥ 2.\n\nExclusion criteria:\n\n* Presence of thrombus in the left atrial appendage.\n* Complications related to the index procedure.\n* Insufficient quality of the left atrial electroanatomical map.",{"count":57,"type":20},50,"OBSERVATIONAL","The goal of this observational study is to analyze the characteristics of left atrial electroanatomical maps in patients without a history of atrial fibrillation but with a high clinical risk of developing it, as indicated by the presence of structural heart disease or a CHA2DS2-VASc score ≥ 2 points. The study cohort will be compared to a historical cohort of patients with diagnosed atrial fibrillation in a propensity-matched fashion.\n\nThe main questions it aims to answer are:\n\n* Are the left atrial electroanatomical changes a consequence or a precursor to the development of atrial fibrillation?\n* Are the left atrial electroanatomical findings different between patients with atrial fibrillation and those at high risk of developing it?\n* What is the prognostic impact of left atrial pathologic changes in patients without diagnosed atrial fibrillation in terms of cardiovascular outcomes?",[28],"RECRUITING","2024-05-21",{"date":64,"type":40},"2024-05-22",{"date":66,"type":40},"2023-11-08",{"date":68,"type":20},"2027-10-31",{"name":70,"class":71},"Universitair Ziekenhuis Brussel","OTHER"]
