Although the pathophysiology of BPPV is mainly attributed to the displacement of otoconia within the semicircular canals, recent studies suggest that psychological stress and anxiety may be associated with the onset or recurrence of the disease. However, the biological mechanisms underlying this relationship remain poorly understood, and objective evaluation of autonomic nervous system function in patients with BPPV is limited.\n\nHeart rate variability (HRV) is a non-invasive method widely used to assess autonomic nervous system activity. Short-term HRV measurements provide information about autonomic balance and vagal tone. In particular, parameters such as the root mean square of successive differences (RMSSD) and the coefficient of variation of RR intervals (COV RR\u002FR) are considered indicators of parasympathetic activity.\n\nThe aim of this prospective case-control study is to evaluate vagal tone in patients with BPPV using short-term HRV analysis and to compare the findings with age-, sex-, and body mass index-matched healthy controls. Participants will undergo standardized HRV measurements at rest and during controlled breathing. Perceived stress levels will also be assessed using the Perceived Stress Scale (PSS-10).\n\nThe primary outcomes of the study are RMSSD and the coefficient of variation of RR intervals (COV RR\u002FR), obtained from short-term HRV recordings. These parameters will be compared between BPPV patients and healthy controls, and their associations with perceived stress levels will also be explored. The findings of this study may provide insights into the role of autonomic regulation and stress-related mechanisms in the pathophysiology of BPPV.",[27,28],"BPPV","Stress and Anxiety",[27,30,31,32,33],"stress","HRV","heart rate variability","Benign paroxysmal positional vertigo","RECRUITING","2026-04-13",{"date":37,"type":38},"2026-04-16","ACTUAL",{"date":40,"type":38},"2026-04-01",{"date":42,"type":23},"2026-09-01",{"name":44,"class":45},"Istanbul Aydın University","OTHER",1,{"id":48,"slug":49,"hasResults":11,"nctId":50,"briefTitle":51,"officialTitle":51,"acronym":4,"eligibilityCriteria":52,"healthyVolunteers":11,"sex":18,"minAge":19,"maxAge":53,"enrollmentInfo":54,"targetDuration":4,"studyType":56,"phases":57,"briefSummary":59,"conditions":60,"keywords":63,"overallStatus":34,"whyStopped":4,"lastUpdateSubmitDate":66,"lastUpdatePostDateStruct":67,"startDateStruct":69,"completionDateStruct":71,"leadSponsor":73,"locationsCount":46},"100613545","additional-effects-of-brandt-daroff-exercises-with-gans-maneuver-on-dizziness-and-quality-of-life-in-patients-with-bppv-100613545","NCT07267988","Additional Effects Of Brandt-Daroff Exercises With GANS Maneuver On Dizziness And Quality Of Life In Patients With BPPV","Inclusion Criteria:\n\n* Diagnosed Patients of Benign Paroxysmal Vertigo (BPPV)\n* Age between 18-60\n* Acute and Chronic BPPV Patients\n* Positive Dix-Hallpike Test\n* DHI Scale: Mild-Severe (0-100)\n\nExclusion Criteria:\n\n* Patients with a history of vestibular surgery or cochlear\n* Patients with a history of seizures or epilepsy\n* Patients with a pacemaker or implantable cardioverter-defibrillator (ICD)\n* Patients with a history of severe head injury or concussion within the past year\n* No other vestibular disorders or conditions that may affect balance (e.g., Meniere's disease, labyrinthitis\n* No musculoskeletal or neurological conditions that may affect balance (e.g., stroke, Parkinson's disease)","60 Years",{"count":55,"type":23},30,"INTERVENTIONAL",[58],"NA","Benign Paroxysmal Positional Vertigo (BPPV) is the most common cause of recurrent vertigo, with a lifetime prevalence of about 2.4% and a yearly incidence of roughly 0.6%. It results from displaced otoconia within the semicircular canals and is especially common in older adults. BPPV leads to brief, position-provoked episodes of vertigo, dizziness, and imbalance, and reduced quality of life. Although canalith-repositioning maneuvers are highly effective, recurrence is common, emphasizing the need for supportive and adjunctive management strategies. This randomized controlled trial will evaluate the Additional Effects OF Brandt-Daroff Exercise With GANS Maneuver On dizziness And Quality of Life In Patients with BPPV. Thirty patients with BPPV will be recruited at Fauji Foundation Hospital and screened using the Dizziness Handicap Inverntory Scale(DHI) and Vestibular Activities and Participation measure(VAP).
