In contrast, low- and middle-income countries (LMICs) carry the global burden of cervical cancer with about 85% of new cases and 90% mortality. Screening is an essential component of effective prevention to reduce this disease burden. The World Health Organization recommends human papillomavirus (HPV) testing as the most effective screening method. However, HPV testing can be expensive and complex to implement. Most tests require central laboratory processing, which means women must come back for a different visit to obtain results and potential treatment. In LMICs, this often results in high loss to follow up because many women face transportation and other challenges. The development of new, low-cost HPV tests that can be processed locally as the potential to improve adherence in screening programs. In this study, the research team will assess the feasibility of a same-day screen-and-treat approach compared to the standard two-visit regime in the context of the cervical cancer prevention program in El Salvador. This will be a cross-sectional study that will enroll 1,000 women in remote areas of the country over 10-15 weeks. In 2020, the World Health Organization launched a global initiative for the worldwide elimination of cervical cancer as a public health issue, setting a target of 70% of women screened with a high-performance test. In Cameroon, this target is far from being reached, and rural communities are disproportionately affected by low uptake of screening services. Recently, a new cervical cancer screening program has launched in the Bafoussam Regional Hospital, in the capital of the Mifi Health District, as part of a larger research project. To develop a context-specific strategy improving access to screening for women living in rural areas of the district, the active participation of community members for the co-development and implementation of local strategies for cervical cancer screening may be key to improving population screening coverage.\n\nObjective - The aim of this study is to evaluate the use of community co-produced action (CCA) for the development and implementation of HPV-based cervical cancer screening strategies compared to a traditional hospital-based approach in rural areas of the Mifi Health District in Cameroon.\n\nMethods - Using a cluster-randomized study design, participatory workshops with community members will be led in 7 rural health areas assigned to the intervention arm, with the aim of codeveloping local strategies for HPV-based cervical cancer screening adapted to the setting. The co-produced strategies may include raising awareness among eligible women (aged 30-49 years, or 25-49 years if HIV-positive), as well as facilitating access to screening tests, results and follow-up. Women living in one of the 7 health areas assigned to the control arm will be invited by community health workers to undergo screening at the Bafoussam Regional Hospital. In both study arms, screened women will be included in the GENOVA study, a cervical cancer screening trial offering free-of- charge HPV testing followed by triage by visual inspection or genotyping for HPV-positive women, and free treatment according to triage results. CCA will be evaluated by assessing its effectiveness for screening uptake. Screening coverage among eligible women will be estimated during one year after implementation of screening strategies in both study arms, based on population statistics provided by the Mifi health district for each health area.\n\nExpected results - Developing context-sensitive solutions to cervical cancer screening through a community-based participatory approach in rural areas of the Mifi health district is expected to improve screening participation rate.",[29],"2026-04-08",{"date":309,"type":39},"2026-04-14",{"date":311,"type":39},"2024-04-13",{"date":313,"type":23},"2027-02",{"name":315,"class":46},"Prof. Patrick Petignat",{"id":317,"slug":318,"hasResults":12,"nctId":319,"briefTitle":320,"officialTitle":321,"acronym":322,"eligibilityCriteria":323,"healthyVolunteers":55,"sex":56,"minAge":57,"maxAge":20,"enrollmentInfo":324,"targetDuration":4,"studyType":24,"phases":326,"briefSummary":327,"conditions":328,"keywords":329,"overallStatus":35,"whyStopped":4,"lastUpdateSubmitDate":336,"lastUpdatePostDateStruct":337,"startDateStruct":339,"completionDateStruct":341,"leadSponsor":343,"locationsCount":47},"100461238","using-artificial-intelligence-based-chatbot-to-improve-womens-participation-to-cervical-cancer-screening-programme-100461238","NCT05286034","Using Artificial Intelligence-based ChatBot to Improve Women's Participation to Cervical Cancer Screening Programme","Artificial Intelligence-assisted Decision-making to Improve Women's Participation to Cervical Cancer Screening in Occitanie Region-France","AppDate-You","Inclusion criteria: eligible women will be:\n\n* aged 30-65\n* did not perform a pap smear in the last 4 years\n* are living in deprived clusters in the Occitanie region.\n* did not answer to a first \"invitation\" letter to perform a clinician-collected HPV testing\n\nExclusion criteria: ineligible women will be those:\n\n* outside the target age group\n* had a Pap smear in the past 3 years\n* had hysterectomy including cervix\n* are pregnant\n* already scheduled a screening appointment or had just attended a HPV screening\n* had a cervical abnormality that was under exploration and\u002For treatment.\n* participated to the study pilot",{"count":325,"type":23},3000,[26],"This project looks to improve the return rate of HPV self-sampling (HPVss) as well as the management of women HPVss positive.",[29,113],[330,331,332,333,334,335],"cervical neoplasia","cervical cancer screening","chatbot","artificial intelligence","cluster randomized controlled trial","HPV vaginal self-sampling","2026-03-25",{"date":338,"type":39},"2026-03-27",{"date":340,"type":39},"2025-11-03",{"date":342,"type":23},"2026-08-18",{"name":344,"class":46},"International Agency for Research on Cancer",{"id":346,"slug":347,"hasResults":12,"nctId":348,"briefTitle":349,"officialTitle":350,"acronym":4,"eligibilityCriteria":351,"healthyVolunteers":55,"sex":56,"minAge":153,"maxAge":20,"enrollmentInfo":352,"targetDuration":4,"studyType":24,"phases":354,"briefSummary":355,"conditions":356,"keywords":360,"overallStatus":64,"whyStopped":4,"lastUpdateSubmitDate":365,"lastUpdatePostDateStruct":366,"startDateStruct":368,"completionDateStruct":369,"leadSponsor":371,"locationsCount":4},"100628469","effect-of-virtual-reality-on-pain-and-anxiety-during-pap-smear-testing-100628469","NCT07462039","Effect of Virtual Reality on Pain and Anxiety During Pap Smear Testing","The Effect of Virtual Reality Transition on Pain and Anxiety Experienced During Pap Smear Testing","Inclusion Criteria:\n\n* Women aged 18-65 years\n* Scheduled for a Pap smear procedure\n* Able to communicate in Turkish\n* Able to understand and complete the study questionnaires\n* Willing to provide written informed consent\n* No visual or hearing impairment preventing the use of a VR headset\n\nExclusion Criteria:\n\n* History of severe motion sickness or vertigo\n* Diagnosed psychiatric disorder affecting anxiety assessment\n* Use of anxiolytic or sedative medication within 24 hours prior to the procedure\n* Presence of acute pelvic pain or active vaginal infection\n* Previous participation in this study\n* Any condition preventing safe use of a VR headset",{"count":353,"type":23},60,[26],"This randomized controlled pretest-posttest experimental study aims to evaluate the effect of virtual reality (VR) on pain and anxiety levels experienced during Pap smear sampling. The findings were intended to lay the groundwork for the Ukrainian Ministry of Health in establishing a structured national screening program. However, all efforts were halted due to the nearby armed conflict, situated just 40 kilometers from the border.\n\nThe World Health Organization's goal to eliminate cervical cancer globally has a gap when it comes to managing cancer control during crises like armed conflicts. We propose a demonstration project to assess whether a simpler, yet modern, cervical cancer control strategy (based on using self-sampling for HPV detection) could also be effective for cervical cancer screening in Zaporizhzhia.\n\nWomen aged 30-60 years who have not had a recent negative HPV test will be invited to participate. Participants receive a self-sampling kit from their primary care provider and can return the sample free of charge to the clinic or community volunteers. All samples are analyzed in accredited laboratories in Zaporizhzhia.\n\nHPV-positive women will receive follow-up care according to national guidelines, including referral to gynecologists for additional tests and treatment if needed. HPV-negative women will be reassured and advised on future screening intervals.\n\nThe study also evaluates how well the screening program can be implemented during conflict conditions. This includes measuring women's acceptance of self-sampling, the willingness of providers to adopt the procedures, and whether the screening process is feasible, practical, and sustainable. Additional process evaluation will explore how the program adapts to challenges such as migration, disrupted health services, and safety concerns.\n\nThis project is conducted through collaboration between Zaporizhzhia State Medical and Pharmaceutical University, the Charitable Foundation \"World Against Cancer,\" and Karolinska Institutet in Sweden, which provides quality assurance support for laboratory procedures. Cervical cancer (CC) is a leading cause of death among women living with HIV (WLHIV) in resource-limited settings. Yet, effective methods for screening and preventing CC are available. The recommanded approach for CC screening is based on multiple steps, including initial test to detect human papillomavirus (HPV) infection, visual inspection to identify women with HPV at risk for precancerous lesion and treatment when required.\n\nDropout may occur at these different steps, compromising the success of the CC elimination strategy. Performing all the screening and treatment sequences in a single visit has been recommanded based on the results of a large South African trial. Yet, in many contexts, including those with limited resources, the screening and treatment activities are performed in multiple visites for logistical reasons, resulting in many dropouts.\n\nDifferent strategies for delivering screening with HPV testing for WLHIV are possible. A first approach (\"centralized approach\") consists of having well equipped reference centres with experienced health workers and referring women to these centers. An alternative consists of having a mobile unit who can bring equipment and health workers and perform the CC screening in the usual places of patient care (\"decentralised\" or mobile team approach). Each of these two approaches has advantages and limitations in terms of coverage, completeness, cost and quality of screening. It is necessary to evaluate them in real life to inform national decision-makers on the best strategy to use in their countries.\n\nThe OptiTri-MU study aims to evaluate and compare the effectiveness of these two strategies for delivering CC screening (\"centralized\" screening versus \"decentralized\" screening). It will also assess the implementation of each strategy and include three sub-studies designed to evaluate :\n\n* the performance of urinary HPV testing.\n* the performance of different methods to identify women requiring a treatment.\n* the risk of post-treatment cervical disease. Universal screening has the potential to decrease the overall burden of cervical cancer. However, more than half of the individuals who develop cervical cancer have not been screened in more than 5 years, and 20% did not follow-up after an abnormal result.\n\nTo address this problem, the investigators will adapt and test an intervention to improve cervical screening and follow-up in three different settings: Northeastern US urban inner city (Boston), Southeastern US suburban (Florida), and remote west, rural (Hawaii). Our research group previously developed a successful intervention to improve primary cervical cancer prevention, which will be adapted for cervical cancer screening. The intervention combines education and quality improvement methods using a mechanism called Performance Improvement Continuing Medical Education. This type of intervention is attractive to clinicians because they are able to improve their cancer prevention practices while fulfilling requirements for maintaining board certification.\n\nWhen applied to primary prevention, the intervention improved rates by over 10 percentage points, and gains continued for over 4 years after the intervention concluded. The goal of the proposed research is to adapt the intervention to cervical cancer screening, and to evaluate its effectiveness in improving cervical cancer screening and follow-up after abnormal results.\n\nThe proposed intervention represents an innovative and scalable model for promoting cancer prevention and screening activities by clinicians.
