Both estradiol levels must be \\\u003C 50 pg\u002FmL.At the discretion of the study clinician, subjects with a diagnosis of PCOS may undergo one to two serum estradiol levels, at least one week apart. The estradiol level(s) must be \\\u003C 50 pg\u002FmL\n\nExclusion Criteria:\n\nFor FHA and controls\n\n* Disease other than FHA known to affect bone, including untreated thyroid dysfunction, Cushing's disease, renal failure, diabetes mellitus\n* Use of bisphosphonates in the past year\n* Use of other medications known to affect bone metabolism within 3 months of the study (other than calcium and vitamin D supplementation).\n* Current use of systemic corticosteroids\n* Migraine with aura as classified in IHS Classification ICHD-3.\n* Personal history of or first-degree relative with unprovoked thromboembolism (unless the subject has been tested and ruled out for a hypercoagulable state).\n* Active substance use disorder; currently smokes or vapes\n* History of malignancy or Paget disease of bone\n* Pregnant, planning to become pregnant within 12 months after the end of treatment and\u002For breastfeeding\n\nAdditional exclusion criteria for FHA\n\n* Cardiovascular: History of myocardial infarction or stroke; history of hypertension or use of anti-hypertensive medications\n* Immunodeficiency or taking immunosuppressive therapy\n* Other conditions that can cause oligo-amenorrhea such as PCOS, primary ovarian insufficiency. Participants will be randomly assigned to receive romosozumab or placebo for 6 months. All participants will receive one IV infusion of zoledronate at the 6 month visit. All participants will also receive transdermal estradiol and cyclic progesterone. We will investigate whether participants who receive active romosozumab will demonstrate greater improvements in bone density at one year than those who receive placebo. Take estrogen for 1 year either (i) in its natural form as a patch twice a week (and progesterone by mouth for 12 days of each month), or (ii) in its natural form as a pill daily (and progesterone by mouth for 12 days of each month), or (iii) in a synthetic form as a birth control patch weekly for 3 weeks with 1 week off the patch. You will not be able to choose which form of estrogen you will receive as this will be assigned to you based on a pre-existing randomization sequence (like the flip of a coin)\n2.
