This study focuses on "complex" cancer survivors who have higher CVD risk when diagnosed with cancer because they also have had a diagnosis of 1 or more chronic conditions (e.g., hypertension, diabetes, and/or hyperlipidemia) requiring medication management. This study also aims to learn about the ease of implementing the health system intervention from the perspectives of cancer teams, primary care teams, and complex cancer patients.

The main questions the study aims to answer are:

* Do patients enrolled in the PC2 arm remain connected to their primary care teams during active cancer treatment for chronic disease management compared to patients in usual care?
* Do patients enrolled in the PC2 arm have better management of their chronic conditions during active cancer treatment compared to patients in usual care?
* How do the care team and patients experience this change in care delivery related to their work and care experiences?

This study will compare complex cancer survivors who receive care according to the PC2 intervention to usual care to see if PC2 works to improve cardiovascular risk management.

Participants will:

* receive educational materials about the study upon enrollment
* complete on-line or written surveys at 4 times
* Visit the clinic for check ups and test related to the study 4 times",[68,69,70,71,72,73,36,74],"Cancer","Breast Cancer Early Stage Breast Cancer (Stage 1-3)","Bladder Cancer","Prostate Cancer (Adenocarcinoma)","Endometrial Cancer","Hypertension","Diabetes Mellitus",[76,77,78,79,80,81,82,83],"primary care","cancer","cardiovascular","shared care","care coordination","implementation","cancer multi-team system","complex cancer survivor","2026-03-20
2026-03-23
2026-04-15
2029-04-30
Rutgers, The State University of New Jersey",1
"100576756","phase-4-efficacy-and-safety-of-atorvastatin-and-ezetimibe-1010mg-fixed-dose-combination-versus-atorvastatin-20mg-monotherapy-in-bangladeshi-population-100576756","NCT06789432","Efficacy and Safety of Atorvastatin and Ezetimibe (10/10mg) Fixed Dose Combination Versus Atorvastatin (20mg) Monotherapy in Bangladeshi Population","Inclusion Criteria:

* Men and women aged \\>18 years.
* Patients with a confirmed diagnosis of elevated LDL-C levels necessitating medical management.
* Patients in the low to moderate cardiovascular disease (CVD) risk category according to ESC guidelines (2019)

 * Low-risk: Increased LDL-C level without any co-morbidities
 * Moderate-risk: Young patients (T1DM <35 years; T2DM <50 years) with DM duration <10 years, without other risk factors.

Exclusion Criteria:

* History of hypersensitivity to any study drugs.
* Clinically significant hepatic impairment (ALT, AST level \\> 2xULN) and/or renal impairment (Serum creatinine level ≥2xULN).
* Patients with hepatitis B virus (HBV) or hepatitis C virus (HCV) with cirrhosis, end-stage liver disease, cancer, psychiatric instability, HIV + status, intestinal malabsorption.
* Pregnant or lactating females.
* The presence of a condition that, in the opinion of the investigator, would place the subject at increased risk from study participation.
500,[103],"PHASE4","The goal of this clinical trial is to evaluate the efficacy and safety of a fixed-dose combination of atorvastatin (10 mg) and ezetimibe (10 mg) compared to atorvastatin (20 mg) monotherapy in the Bangladeshi population.

Researchers will compare atorvastatin (10 mg) and ezetimibe (10 mg) to atorvastatin (20 mg) monotherapy to see if atorvastatin (10 mg) and ezetimibe (10 mg) FDC works to treat dyslipidemia.

Participants will:

* Take a fixed-dose combination of atorvastatin (10 mg) and ezetimibe (10 mg) compared to atorvastatin (20 mg) monotherapy for 3 months
* Follow-up visits at 6 weeks and 12 weeks for checkups and tests",[36],[107,108,109],"increased LDL-C level","Hyperlipidemia","Atorvastatin (10 mg) and Ezetimibe (10 mg)","RECRUITING","2025-07-25
2025-07-29
2025-02-01
2025-12
Dr. Md.
