Although extraintestinal manifestations are common in IBD, renal involvement remains relatively underrecognized and insufficiently studied.\n\nThe study will recruit approximately 6,000 participants, including 3,000 patients with confirmed IBD (Crohn's disease or ulcerative colitis) and 3,000 individuals without IBD serving as a control group. Participants will undergo routine clinical assessment, including laboratory tests, urinalysis with measurement of the albumin-to-creatinine ratio (ACR), and imaging evaluation of the kidneys and urinary tract. Disease activity in patients with IBD will be assessed using validated clinical indices.\n\nThe study will also evaluate the relationship between renal dysfunction and disease activity, medications used in IBD treatment, and comorbidities. The results of this study are expected to improve the understanding of renal complications in patients with IBD and support earlier identification and management of kidney disease in this population.",[27,28,29],"IBD and Renal Involement","Chronic Kidney Disease","Inflamatory Bowel Disease (Crohn's and Ulcerative Colitis)",[31,32,33,34,28,35,36,37],"IBD","Crohn's Disease","Ulcerative Colitis","Renal Dysfunction","Extraintestinal Manifestations","Renal Complications in IBD","Albumin-to-Creatinine Ratio","RECRUITING","2026-05-13",{"date":41,"type":42},"2026-05-18","ACTUAL",{"date":44,"type":42},"2025-11-20",{"date":46,"type":23},"2026-12-30",{"name":48,"class":49},"Państwowy Instytut Medyczny Ministerstwa Spraw Wewnętrznych i Administracji","OTHER_GOV",1,{"id":52,"slug":53,"hasResults":11,"nctId":54,"briefTitle":55,"officialTitle":55,"acronym":4,"eligibilityCriteria":56,"healthyVolunteers":11,"sex":18,"minAge":19,"maxAge":57,"enrollmentInfo":58,"targetDuration":60,"studyType":24,"phases":4,"briefSummary":61,"conditions":62,"keywords":4,"overallStatus":38,"whyStopped":4,"lastUpdateSubmitDate":64,"lastUpdatePostDateStruct":65,"startDateStruct":67,"completionDateStruct":69,"leadSponsor":71,"locationsCount":50},"100630423","a-prospective-observational-study-on-the-efficacy-and-safety-of-guselkumab-in-the-treatment-of-crohns-disease-patients-previously-treated-with-ustekinumab-100630423","NCT07487480","A Prospective Observational Study on the Efficacy and Safety of Guselkumab in the Treatment of Crohn's Disease Patients Previously Treated With Ustekinumab","Inclusion Criteria:\n\n* Age 18-75 years.\n* Diagnosis of Crohn's disease for at least 3 months.\n* Moderate to severe disease activity (CDAI 220-450).\n* Previous treatment with at least one standard dose of ustekinumab with documented:\n* Primary non-response, or Secondary loss of response, or Intolerance leading to discontinuation.\n* Willing and able to comply with all study visits and procedures.\n\nExclusion Criteria:\n\n* Previous exposure to any anti-IL-23p19 therapy (including guselkumab).\n* Active intestinal infection, unremoved stenotic lesions with obstructive symptoms, short bowel syndrome, stoma, or recent abdominal abscess (\\\u003C8 weeks).\n* History of malignancy (except certain skin cancers), active tuberculosis, or severe opportunistic infection.\n* Pregnancy, lactation, or planned pregnancy during the study period.","75 Years",{"count":59,"type":23},60,"2 Years","This is a prospective, single-center, observational study aimed at evaluating the efficacy and safety of guselkumab in patients with moderate to severe Crohn's disease (CD) who have previously failed or were intolerant to ustekinumab therapy.
