Confirmed diagnosis of liver cirrhosis (clinical, laboratory, imaging)\n2. Splenomegaly and hypersplenism\n3. No history of portal hypertension bleeding (esophageal and gastric variceal bleeding )\n4. Age 18-80 years, male or female\n5. Child-Pugh Class A or B liver function\n6. No history of primary renal disease or acute kidney injury (AKI)\n7. Signed written informed consent\n8. Ability to complete 24-month follow-up\n\nExclusion Criteria:\n\n1. Child-Pugh Class C liver cirrhosis\n2. Primary renal diseases (glomerulonephritis, polycystic kidney disease, chronic pyelonephritis, etc.)\n3. Previous abdominal surgery precluding safe laparoscopic splenectomy\n4. Severe cardiac, pulmonary, cerebrovascular dysfunction; malignant tumors; primary hematological disorders\n5. Laparoscopic splenectomy is commonly used to treat splenomegaly and hypersplenism in these patients, but its impact on kidney function over 2 years is unclear. Kidney function will be measured by laboratory parameter using urine sample (KIM-1 urine) and blood sample (serum creatinine). The main questions it aims to answer are:\n\n1. Does N-Acetylcysteine lower the level of KIM-1 (Kidney Injury Molecule) in patients urine indicating kidney function protection?\n2. Does N-Acetylcysteine lower the level of creatinine in patients blood indicating kidney function protection?\n\nParticipants will:\n\n1. Had their blood and urine sample taken before taking the drugs (N-Acetylcysteine or placebo)\n2. Underwent cisplatin based chemotherapy\n3. Taken placebo or N-Acetylcysteine for seven days (1 day before chemotherapy, on the chemotherapy day, and 5 days after chemotherapy)\n4. Had their blood and urine sample taken twice after taking the drugs (1 week and 3 weeks after chemotherapy)\n5. Had their symptoms monitor during and after taking the drugs.
