The investigators hope to answer the following questions:\n\n1. How many LTRF treatments are needed to achieve the desired shape in a given cartilage specimen\n2. How does an LTRF treatment quantitatively affect both length and angulation of a cartilage specimen\n3. How does physician satisfaction with cosmetic outcome compare to cases treated with existing septorhinoplasty techniques\n4. How do changes in patient reported measures of nasal obstruction (NOSE score) pre- and post-surgery compare to cases treated with existing septorhinoplasty techniques\n5. Does using LTRF during septorhinoplasty significantly increase total operative time\n6. Are rates of adverse events when using LTRF different from cases treated with existing septorhinoplasty techniques\n\nParticipants will complete the NOSE questionnaire at their pre-operative visit. Participants undergoing cosmetic septhorhinoplasty will also be photographed in standard fashion for facial analysis.\n\nDuring scheduled septorhinoplasty surgery, the Vivaer LTRF device will be used to reshape nasal and\u002For auricular cartilage ex-vivo prior to reimplantation at the discretion of the Principal Investigator in order to optimize functional and cosmetic outcome.\n\nParticipants will follow up at approximately 1 week and 1 month after surgery. A Pragmatic Randomized Controlled Trial","Inclusion Criteria:\n\n* Patients should satisfy all the following criteria to be considered eligible for randomization:\n\n  1. Be age 18 or above\n  2. Able to provide written informed consent\n  3. Have an indication for batten graft, septoplasty and turbinate reduction according to prevailing surgical practices.\n\n  \u003C!-- -->\n\n  1. Septal deviation must be present on direct or endoscopic examination\n  2. Inferior turbinate hypertrophy must be present, direct examination or endoscopic examination\n  3. Collapse of external nasal valve and\u002For lateral motion instability must be documented\n\n  \u003C!-- -->\n\n  1. The ENV maye be assessed clinically by observing the alar collapse at baseline or with forced inspiration, Modified cottle may also be performed.\n\nIn all patients, endoscopic examination should document that the (a) septal deviation, (b) turbinate hypertrophy, and (c) external nasal valve collapse are the primary contributing factors of obstructed breathing.\n\nExclusion Criteria:\n\n1.
