The intervention occurs immediately following an appointment where a patient receives a diagnosis of an ambulatory sensitive condition (ASC; i.e., a condition considered to be a risk factor for near-term ED use). The evaluation will compare eligible patients with an ASC who were randomly assigned to receive follow-up outreach (patient portal message and/or call) from a nurse (who was automatically prompted via the Epic electronic health record system to initiate outreach) with those who were randomly assigned to receive standard care. Analyses will be intent-to-treat. The primary outcome is ED use in the week (i.e., 7 days) following the appointment.

We ran an earlier version of this intervention (NCT06798389). The current study is modified based on results and clinical guidance. Specifically, more conditions will be included as qualifying ASCs for enrollment. Patients under 30 will be excluded. And rather than calling all patients as in the original study, patient portal users may be contacted via the portal instead of or in addition to a phone call. Finally, in the first study, the intervention was differentially effective by age group (<45, 45-64, 65+). Our primary analysis will be conducted separately by age group, though we will also conduct an analysis combining across age groups.

We will run the study until we reach at least 4,330 patients in each of the following age groups: patients aged 30-45, patients aged 45-64, patients aged 65+. Therefore, our estimated sample size is at least 4,330x3 = 12,990.

We may be required to do an interim data pull and/or stop the study early at the direction of clinical or operational leaders.",[26,27,28],"Behavioral Intervention","Emergency Department Visits","Nurse Care Coordination",[30],"Nudge","RECRUITING","2026-02-10
2026-02-11","ACTUAL
2025-05-29
2026-07-31
Geisinger Clinic","OTHER",1
"100612246","dyadic-app-supported-collaborative-care-intervention-trial-for-family-caregivers-of-people-living-with-dementia-100612246","NCT07251088","Dyadic, App-supported Collaborative Care Intervention Trial for Family Caregivers of People Living With Dementia","Dyadic, App-supported Collaborative Care Intervention Trial for Family Caregivers of People Living With Dementia - The Multi-center Living@Home Study","Living@Home","Inclusion Criteria:

* formal dementia diagnosis
* living in their own home for the person with dementia
* access to a smartphone and internet for the informal caregiver
* informed consent of the participant

Exclusion Criteria:

* not able to provide written consent
* if a legal representative is present, they may give their consent
554,[23],"In Germany, approximately 1.8 million individuals are living with dementia, representing a considerable share of those requiring long-term care. Many people with dementia (PlwD) express the desire to remain in their home environment for as long as possible. However, the progressive cognitive and physical decline associated with the disease renders caregiving increasingly time-intensive and places a substantial burden on family members. In the absence of sufficient support structures, maintaining home-based care becomes difficult, creating additional strain on the health care system.

The study aims to address these challenges through an innovative intervention. Its primary objectives are: (1) to evaluate whether a dyadic care management model, delivered by specialized nurses with expertise in dementia care and supported by a mobile health application that provides direct access to caregiving experts and memory clinics, can help stabilize the home care situation; (2) to determine whether this approach reduces caregiver burden; and (3) to assess its effectiveness in alleviating neuropsychiatric symptoms in PlwD, compared to usual care.",[57,28,58],"Dementia","Informal Caregiver",[57,60,61,62,63],"Dementia Care","Caregiver of people with dementia","Dyade","digital health intervention","NOT_YET_RECRUITING","2025-11-18
2025-11-26
2026-01-01
2028-12
German Center for Neurodegenerative Diseases (DZNE)",12]
