These fractures are common in older adults and can significantly impact mobility, independence, and quality of life. There is currently no clear evidence to determine which treatment is better for this population. The main questions it aims to answer are:\n\n* Does surgery or conservative collar treatment lead to better improvement in daily living, measured by the Barthel Index, after 12 weeks?\n* What are the differences in pain, disability, and quality of life between the two treatments?\n\nThe study team will compare patients who receive surgical stabilization with those who are treated with a rigid cervical collar to see which approach supports better functional outcomes and healing.\n\nParticipants will:\n\n* be randomly assigned to either surgery (posterior C1-C2 screw-rod fixation) or conservative collar treatment\n* attend study visits at 12 weeks and 6 months\n* complete questionnaires on daily functioning, pain, and quality of life\n* undergo CT scans and other medical assessments\n* record collar use (for conservative group) in a diary\n* be monitored for any complications or changes in treatment (including crossover to surgery if needed)\n\nThe study aims to include 322 participants to provide evidence on which treatment helps older adults recover better from odontoid fractures with fewer complications and improved quality of life.",[26,27],"Odontoid Fracture","Elderly",[29,30,31,32,33,34],"Odontoid fracture","C2 fracture","atlantoaxial instability","atlantoaxial fusion","Barthel Index","functional outcome","RECRUITING","2026-05-05",{"date":38,"type":39},"2026-05-06","ACTUAL",{"date":41,"type":39},"2025-08-13",{"date":43,"type":20},"2028-06-01",{"name":45,"class":46},"Technical University of Munich","OTHER",16,{"id":49,"slug":50,"hasResults":11,"nctId":51,"briefTitle":52,"officialTitle":53,"acronym":4,"eligibilityCriteria":54,"healthyVolunteers":11,"sex":16,"minAge":4,"maxAge":4,"enrollmentInfo":55,"targetDuration":4,"studyType":57,"phases":4,"briefSummary":58,"conditions":59,"keywords":60,"overallStatus":64,"whyStopped":4,"lastUpdateSubmitDate":65,"lastUpdatePostDateStruct":66,"startDateStruct":68,"completionDateStruct":70,"leadSponsor":72,"locationsCount":4},"100584499","outcomes-of-fracture-odontoid-management-100584499","NCT06890130","Outcomes of Fracture Odontoid Management","Long-Term Outcomes of Fracture Odontoid Management in Assiut University","Inclusion Criteria:\n\n* All patients with odontoid fractures presenting to Assiut University Hospital - Department of Orthopaedic and Trauma Surgery who have completed a minimum of 2 year follow up regardless of age, mechanism of injury, neurological status or type of treatment applied.\n\nExclusion Criteria:\n\n* • Patients who are not available for two year follow-up\n\n * Patients who refuse to participate in the study",{"count":56,"type":20},20,"OBSERVATIONAL","This study aims to evaluate the long-term outcomes of treating patients with odontoid fractures admitted to the Trauma Unit of Assiut University Hospital.",[26],[61,62,63],"odontoid fracture","management","long term outcomes","NOT_YET_RECRUITING","2025-03-17",{"date":67,"type":39},"2025-03-21",{"date":69,"type":20},"2025-05-01",{"date":71,"type":20},"2026-12-30",{"name":73,"class":46},"Assiut University"]
