Dysphagia in PD is associated with aspiration pneumonia, malnutrition, and impaired medication intake, which together represent one of the leading causes of morbidity and premature mortality in these patients. Despite its clinical relevance, the underlying biological mechanisms of dysphagia in PD are not fully understood, and current treatment strategies are limited.\n\nThe purpose of this study is to investigate the clinical, biological, and neural determinants of oropharyngeal dysphagia in patients with PD, and to explore compensatory mechanisms of the brain that may counteract swallowing difficulties. We hypothesize that dysphagia in PD is linked not only to disease severity and progression but also to specific biological markers and neural plasticity in the swallowing network.\n\nThis is a prospective, cross-sectional observational study including 100 patients with PD. Swallowing function will be systematically assessed using flexible endoscopic evaluation of swallowing (FEES), a gold standard method for detecting penetration and aspiration. Additional clinical data will be collected, including motor and non-motor symptoms, disease severity, and quality of life measures. Biological assessments will include blood-based biomarkers related to inflammation and neurodegeneration. Furthermore, functional magnetic resonance imaging (fMRI) will be used to examine cortical and subcortical activity patterns associated with swallowing and to identify potential compensatory activation in dysphagic and non-dysphagic patients.\n\nBy integrating clinical, biological, and imaging approaches, this study aims to provide a comprehensive characterization of dysphagia in PD. The findings are expected to improve the understanding of disease mechanisms and to identify predictors of dysphagia onset and severity. Ultimately, this knowledge may help to guide the development of targeted therapeutic strategies, reduce the risk of severe complications, and improve quality of life for patients with Parkinson's disease.",[101,25],"Parkinsons Disease (PD)",[103],"Dysphagia, Swallowing Disorder, Parkinson's Disease, Swallowing Compensation, Neuroimaging Biomarkers, Functional MRI, Proteomics","2025-12-09",{"date":106,"type":34},"2025-12-23",{"date":108,"type":34},"2025-09-01",{"date":110,"type":20},"2028-01",{"name":112,"class":41},"Heinrich-Heine University, Duesseldorf",{"id":114,"slug":115,"hasResults":11,"nctId":116,"briefTitle":117,"officialTitle":117,"acronym":4,"eligibilityCriteria":118,"healthyVolunteers":11,"sex":16,"minAge":50,"maxAge":4,"enrollmentInfo":119,"targetDuration":4,"studyType":121,"phases":122,"briefSummary":124,"conditions":125,"keywords":129,"overallStatus":30,"whyStopped":4,"lastUpdateSubmitDate":133,"lastUpdatePostDateStruct":134,"startDateStruct":136,"completionDateStruct":138,"leadSponsor":140,"locationsCount":65},"100596745","the-effect-of-swallowing-exercise-and-maneuver-program-for-older-adults-with-dysphagia-in-nursing-home-100596745","NCT07049471","The Effect of Swallowing Exercise and Maneuver Program For Older Adults With Dysphagia in Nursing Home","Inclusion Criteria:\n\n* aged 60 years or older\n* experiencing swallowing difficulties as assessed by the Eating Assessment Tool-10 (EAT-10)\n* having no history of neurological disease\n* possessing adequate communication abilities\n* and being capable of performing daily activities either independently or with partial assistance\n\nExclusion Criteria:\n\n* individuals with dermatological conditions in the anterior neck region, hypertension, orofacial pain such as trigeminal neuropathy or toothache, as well as those with significant malocclusion or facial asymmetry",{"count":120,"type":20},72,"INTERVENTIONAL",[123],"NA","This clinical trial aimed to evaluate the effectiveness of a structured swallowing exercise and maneuver program for older adults with dysphagia living in a nursing home. Dysphagia, or difficulty in swallowing, is common in elderly populations and may lead to malnutrition, dehydration, and aspiration pneumonia.\n\nIn this study, 72 participants aged 60 years and older were randomly assigned to either an intervention group or a control group. The intervention group received a combined swallowing exercise and maneuver program-including Masako Maneuver, Chin Tuck Against Resistance, Supraglottic Swallow, and Super-Supraglottic Swallow-performed three times daily before meals over a six-week period. The control group received standard care, including upright sitting posture during meals.\n\nSwallowing ability was measured using the EAT-10 questionnaire.
