Many patients present with obstructive jaundice and undergo preoperative percutaneous transhepatic cholangial drainage (PTCD) to relieve biliary obstruction. However, there is currently no consensus on whether the PTCD catheter should be removed or retained during surgery.\n\nThis multicenter, prospective randomized controlled trial aims to compare two intraoperative strategies: removal versus retention of the PTCD catheter during PD. Participants will be randomly assigned to either group. The study will evaluate whether these different approaches influence postoperative outcomes, particularly major complications such as bile leak and severe postoperative morbidity within 90 days after surgery.\n\nIn addition to complications, the study will assess recovery after surgery, including return of gastrointestinal function, length of hospital stay, and quality of recovery, as well as laboratory indicators of liver function and inflammation.\n\nThe results of this study are expected to provide evidence to guide surgical decision-making regarding PTCD management during PD and to improve patient outcomes.",[53,54,26,55],"Obstructive Jaundice","Pancreatic Head Cancer","Biliary Tract Neoplasms",[57,58,59,60,61,62,63],"Pancreaticoduodenectomy","PTCD","Biliary drainage","Biliary complications","Bile leak","Enhanced recovery","Randomized controlled trial","2026-04-09",{"date":66,"type":32},"2026-04-16",{"date":68,"type":20},"2026-05-01",{"date":70,"type":20},"2028-04-30",{"name":72,"class":39},"Xu'an Wang",{"id":74,"slug":75,"hasResults":11,"nctId":76,"briefTitle":77,"officialTitle":78,"acronym":79,"eligibilityCriteria":80,"healthyVolunteers":11,"sex":16,"minAge":17,"maxAge":4,"enrollmentInfo":81,"targetDuration":4,"studyType":83,"phases":4,"briefSummary":84,"conditions":85,"keywords":4,"overallStatus":28,"whyStopped":4,"lastUpdateSubmitDate":87,"lastUpdatePostDateStruct":88,"startDateStruct":90,"completionDateStruct":92,"leadSponsor":94,"locationsCount":96},"100625094","endoscopic-management-of-biliopancreatic-disease-in-the-lombardy-pancreas-units-100625094","NCT07418164","Endoscopic Management of Biliopancreatic Disease in the Lombardy Pancreas Units","Endoscopic Management of Suspected Biliopancreatic Disease Within the Lombardy Pancreas Units: a Prospective Observational Study","GENTLENESS","Inclusion Criteria:\n\n* Adults (≥18 years)\n* Clinical, biochemical, and\u002For imaging suspicion of pancreatic neoplasia (adenocarcinoma, neuroendocrine tumors, cystic neoplasms requiring endoscopic, surgical, or oncological treatment) or periampullary neoplasia (distal bile duct, ampulla, duodenum)\n* Candidate for EUS according to standard clinical practice\n* Ability to understand and sign informed consent\n\nExclusion Criteria:\n\n* Age \\\u003C18 years\n* Pregnancy or breastfeeding\n* Conditions other than those defined in the inclusion criteria\n* Inability to provide informed consent\n* Refusal to participate in the study",{"count":82,"type":20},2500,"OBSERVATIONAL","The GENTLENESS study is a prospective, multicenter, observational registry designed to evaluate the endoscopic management of patients with suspected biliopancreatic and periampullary neoplastic diseases within the Pancreas Units network of the Lombardy Region.\n\nPatients undergoing endoscopic ultrasound (EUS) for suspected pancreatic or periampullary malignancies, including pancreatic ductal adenocarcinoma, pancreatic neuroendocrine tumors, cystic pancreatic neoplasms requiring endoscopic, surgical, or oncological management, and periampullary tumors of the distal bile duct, ampulla, or duodenum, will be prospectively enrolled according to standard clinical practice.\n\nThe study aims to assess the diagnostic accuracy of EUS-guided tissue acquisition, procedural safety, the need for repeat sampling, and organizational performance indicators, including time to definitive cyto-histological diagnosis and time to initiation of disease-specific treatment.
