This is believed to be due to hormonally induced changes in the vaginal flora associated with menopause. After menopause, there is a chemical changes in the vagina that may predispose to bacterial infections.\n\nThe role of vaginal estrogen creams to restore vaginal atrophy and prevent urinary tract infections has been well characterized. Vaginal testosterone (VT) application use in postmenopausal breast cancer patients on aromatase inhibitors have been shown to improve vaginal pH, vaginal atrophy symptom scores, dyspareunia, and vaginal dryness. Although testosterone has been used to improve sexual function in postmenopausal women, the effects of VT on vaginal flora and recurrent UTIs are unknown.\n\nThe purpose of this study is to determine whether topically applied vaginal testosterone cream is more effective than placebo in reducing the incidence of urinary tract infections in postmenopausal women with recurrent urinary tract infections and to ascertain the effects of topical estrogen on the vaginal pH and flora.",[29,30,31],"Recurrent Urinary Tract Infection","Vaginal Atrophy","Postmenopausal Disorder",[33,34,35,36],"urinary tract infections,","postmenopausal women","vaginal atrophy,","vaginal flora","RECRUITING","2025-11-04",{"date":40,"type":41},"2025-11-05","ACTUAL",{"date":43,"type":41},"2020-11-01",{"date":45,"type":23},"2026-12-31",{"name":47,"class":48},"Maimonides Medical Center","OTHER",1,{"id":51,"slug":52,"hasResults":11,"nctId":53,"briefTitle":54,"officialTitle":55,"acronym":56,"eligibilityCriteria":57,"healthyVolunteers":11,"sex":18,"minAge":58,"maxAge":59,"enrollmentInfo":60,"targetDuration":4,"studyType":24,"phases":62,"briefSummary":64,"conditions":65,"keywords":74,"overallStatus":37,"whyStopped":4,"lastUpdateSubmitDate":84,"lastUpdatePostDateStruct":85,"startDateStruct":87,"completionDateStruct":89,"leadSponsor":91,"locationsCount":49},"100588378","a-trial-of-d-mannose-for-the-prophylaxis-of-recurrent-urinary-tract-infections-100588378","NCT06940622","A Trial of D-mannose for the Prophylaxis of Recurrent Urinary Tract Infections","A Randomized Controlled Trial of D-mannose for the Prophylaxis of Recurrent Urinary Tract Infections in Post-menopausal Women","DmannoseRCT","Inclusion Criteria:\n\n* Female, post-menopausal, age ≥ 55 years old\n* Diagnosis of recurrent UTI, defined as ≥ 3 symptomatic culture-proven UTI episodes in 12 months or ≥ 2 in 6 months.\n* Currently free from a UTI determined based on absence of symptoms as determined by the UTI symptom assessment questionnaire79 and negative urine culture (\\\u003C103 colony forming units per ml of urine).\n* Able to attend all follow-up appointments for the study.\n* A negative upper and lower urinary tract evaluation, including pelvic examination for pelvic organ prolapse (less than stage 2), measurement of post-void residual (less than 50 ml), and imaging (renal ultrasound and standing voiding cystourethrogram) to exclude kidney stone, hydronephrosis, reflux, or urethral diverticulum.\n\nExclusion Criteria:\n\n* Current use of D-mannose.
