Nitrofurantoin Versus Bladder Fulguration Plus Daily Nitrofurantoin for Women With Recurrent Urinary Tract Infections","A Multicentric Randomized Trial of Daily Nitrofurantoin Versus Bladder Fulguration Plus Daily Nitrofurantoin for the Long-term Management of Cystitis in Women With Recurrent Urinary Tract Infections","Inclusion Criteria:\n\n* Females 18 to 85 years old with at least a 1-year history of culture documented uncomplicated rUTI.\n* Diagnosis of rUTI, defined as ≥ 3 symptomatic UTIs in 12 months or ≥ 2 in 6 months.\n* Currently free from a UTI determined based on absence of symptoms as determined by the UTI symptom assessment questionnaire and negative urine culture (\\\u003C10\\^3 colony forming units per ml of urine).\n* A negative upper and lower urinary tract evaluation, including pelvic examination for pelvic organ prolapse (less than or equal to stage 2), measurement of post-void residual (less than 50 ml), and imaging (which may include renal ultrasound and standing voiding cystourethrogram) to exclude kidney stone, hydronephrosis, reflux, or urethral diverticulum.\n* Office cystoscopy documenting stages 1 or 2 of chronic cystitis.\n* Likely to stay in the geographic region for the duration of the study.\n* ASA class II or less.\n\nExclusion Criteria:\n\n* Patients on antibiotics at baseline (i.e., suppressive therapy or antibiotic therapy for non-urinary infections).\n* Patients on self-start therapy (i.e., taking antibiotics upon start of urinary symptoms concerning for UTI).\n* Patients on prophylactic antibiotics started in the last 3 months and unwilling to discontinue, or intention to start in the next 12 months.\n* Complicated UTIs, including neurogenic bladder condition (i.e., multiple sclerosis, Parkinson's disease, spinal cord injury), bladder augmentation, or urinary diversion.\n* Patients with urinary catheters (including indwelling Foley, intermittent catheterization, and suprapubic catheters).\n* Uncontrolled diabetes (HbA1c \\>9).\n* Pregnancy\n* Allergy or resistance to Nitrofurantoin.\n* Chronic lung or liver condition precluding the use of Nitrofurantoin, including abnormal chest X ray or elevated urinary tract infections (UTIs). Specifically, participants will have had 3 UTIs over the past 12 months or 2 UTIs over the past 6 months. At least one of these UTIs must be culture-proven. For UTIs to count for this study, if not culture-proven they must involve the onset of UTI-related symptoms (dysuria, urgency, frequency, suprapubic pain, or urinary changes, etc) with prescription of antibiotics directed at UTI treatment.\n\nExclusion Criteria\n\n* Chronically self-catheterize.\n* Currently use of a vaginal pessary.\n* Have used a vaginal probiotic in the prior 3 months.\n* Are pregnant or within 1 year postpartum.\n* Have poorly controlled diabetes with an A1c of 9 or greater.\n* Are considered immunocompromised\n* Are unable to personally provide legal consent",{"count":56,"type":21},156,[24],"The objective of this study is to evaluate the use of a vaginal probiotic in patients with recurrent urinary tract infections (UTIs). Specifically, this study aims to evaluate factors associated with successful colonization with Lactobacillus crispatus - a known \"healthy\" and protective bacteria in both the vaginal and urinary microenvironments. The study will recruit patients over age 18 with a clinical diagnosis of recurrent UTIs, and will utilize a commercially available Lactobacillus vaginal probiotic. Participants will use the probiotic product with the commercially approved packaging and regimen for a total of 3 months. Vaginal and urinary samples will be obtained at four different timepoints. Participants will also be asked to complete surveys regarding tolerability and acceptability of the probiotic product. Patient samples will be analyzed in the laboratory to understand the effects of the probiotic on the local microbiome. Survey data and demographic information will be housed within the Research Electronic Data Capture (RedCap) secure server.
