The study compares three groups: one performing aerobic cycling at an intensity that elevates lactate levels, one performing low-intensity exercise, and one receiving health education without exercise.

All participants will complete motor learning tasks and undergo brain-stimulation testing using transcranial magnetic stimulation (TMS) to assess how well the brain responds to training. The goal is to understand whether different types of exercise can improve brain function, movement, and memory, and how the body's response to exercise (like lactate levels) might support brain health.

This research may help identify low-cost, non-invasive interventions-such as targeted exercise-that improve motor and cognitive outcomes in aging and stroke recovery.",[28,29,30,31,32],"Stroke, Chronic","Aging","Cognitive Decline","Motor Dysfunction","Neuroplasticity",[34,35,32,36,37,29,38,39,40],"Transcranial Magnetic Stimulation (TMS)","Cortical Inhibition","Aerobic Exercise","Motor Learning","Stroke Rehabilitation","Cortical Excitability","GABAergic Inhibition","NOT_YET_RECRUITING","2026-04-16
2026-04-17","ACTUAL
2026-05-14
2030-01-01
University of Alabama at Birmingham","OTHER",1
"100484137","phase-2-repetitive-transcranial-magnetic-stimulation-a-form-of-non-invasive-brain-stimulation-as-a-possible-treatment-for-post-stroke-fatigue-100484137","NCT05584124","Repetitive Transcranial Magnetic Stimulation, a Form of Non-invasive Brain Stimulation, as a Possible Treatment for Post-stroke Fatigue.","Neuromodulation for Rehabilitation of Post-Stroke Fatigue: An rTMS Pilot Study","Inclusion Criteria:

* 34 - 80 years of age
* more than 6 months post-stroke
* Clinically present fatigue

Exclusion Criteria:

* Contraindications to MRI/TMS/rTMS (e.g. implanted metal within the head, history of seizures)
* Inability to follow simple three-step instructions
* Diagnosed comorbid neurological conditions (e.g. multiple sclerosis, Parkinson's disease, moderate/severe TBI)
* Pregnancy
* strokes within stimulation sites
* Pregnancy
* Severe hypertension (resting SBP \\> 200, DBP \\> 120)","34 Years","80 Years
60,[66],"PHASE2","This experimental trial is investigating the use of magnetic stimulation of the brain as a possible treatment for post-stroke fatigue in people who have had a stroke more than six months prior to enrolling. Fatigue is a common condition after a stroke and can negatively impact an individual's ability to perform desired social and physical activities. Magnetic stimulation of the brain has been shown to improve signs and symptoms of related psychological (e.g., depression) conditions. Participants in this trial will receive a type of magnetic stimulation called repetitive transcranial magnetic stimulation, or rTMS.
