However, many patients discontinue peritoneal dialysis, often transferring to hemodialysis. Despite the frequency of this transition, little is known about its impact on patients.\n\nOverall, the UPLIFT-PD program aims to fill this gap by studying the transition from peritoneal dialysis to hemodialysis. The specific goal of this mixed methods sub-study is to conduct a national survey to ask patients and caregivers about priorities and preferences when they anticipate a transfer from peritoneal dialysis to hemodialysis. This survey will be co-created by patient-partners and dialysis experts.\n\nResults from this survey will then be used in other phases of UPLIFT-PD program, detailed in other registrations, which will support people facing a transition from peritoneal dialysis to hemodialysis..",[26,27,28,29],"Peritoneal Dialysis (PD)","Hemodialysis","Kidney Replacement Therapy","Transition",[31],"Transfer to hemodialysis","NOT_YET_RECRUITING","2026-02-24",{"date":35,"type":36},"2026-02-27","ACTUAL",{"date":38,"type":22},"2027-01",{"date":40,"type":22},"2029-12",{"name":42,"class":43},"Ciusss de L'Est de l'Île de Montréal","OTHER",{"id":45,"slug":46,"hasResults":11,"nctId":47,"briefTitle":48,"officialTitle":49,"acronym":4,"eligibilityCriteria":50,"healthyVolunteers":11,"sex":18,"minAge":51,"maxAge":52,"enrollmentInfo":53,"targetDuration":4,"studyType":55,"phases":56,"briefSummary":58,"conditions":59,"keywords":4,"overallStatus":61,"whyStopped":4,"lastUpdateSubmitDate":62,"lastUpdatePostDateStruct":63,"startDateStruct":65,"completionDateStruct":67,"leadSponsor":69,"locationsCount":71},"100471008","targeted-interventions-for-successful-transition-and-transfer-of-adolescents-with-inborn-errors-of-metabolism-to-adult-services-100471008","NCT05413278","Targeted Interventions for Successful Transition and Transfer of Adolescents With Inborn Errors of Metabolism to Adult Services","Fit for Transfer: Targeted Interventions for Successful Transition and Transfer of Adolescents With Inborn Errors of Metabolism to Adult Metabolic Services","Inclusion Criteria:Patients\n\n* with an inborn error of metabolism in the care of a specialized metabolic care unit\n* requiring specialized adult metabolic care\n* at least 14 years old\n* with a disease for which at least one biochemical and \u002F or physical parameter disease marker is well established .\n\nExclusion Criteria:\n\nPatients -\n\n* with insufficient knowledge of the German\n* with cognitive impairment to a degree that consent, and participation would be impossible\n* in an end-of-life situation\n* with a disease for which no biochemical and \u002F or physical parameter disease marker is well established","14 Years","25 Years",{"count":54,"type":22},20,"INTERVENTIONAL",[57],"NA","Main aims of this project are\n\n* To assess the baseline status-quo of transition and \"fitness for transfer\" in terms of information about the adult centre and team, organisational and practical skills (blood sampling and sending, how to make an appointment etc.), disease- and treatment-related knowledge, health-related quality of life (HrQoL), and self-efficacy in adolescnets with inborn errors of metabolism.
