It must be noted that the standard healthcare system is hardly used, or can only be used to an insufficient extent, by homeless people, and therefore does not comprehensively reach this group. One low-threshold and acceptance-oriented service outside the standard care system is the Johanniter-Hilfsmobil, which was established on June 3, 2024, as an innovative approach to healthcare for homeless people in Augsburg and will be scientifically examined in the following study.

The aim is to compare acute and chronic wounds in people experiencing housing emergencies who are treated by the Johanniter-Hilfsmobil with those of patients who attend the outpatient wound clinic at the Clinic for Vascular Surgery at Augsburg University Hospital.

In addition, a saliva test will be carried out to estimate the amount and type of substance consumed, and a survey will be conducted among drug-dependent test subjects who present themselves either via the Johanniter-Hilfsmobil or via the substitution outpatient clinic of the Clinic for Psychiatry, Psychotherapy, and Psychosomatics at Augsburg District Hospital.

Another focus of research is the quantitative and qualitative analysis of the psychosocial living situation and environmental influences on users of the Johanniter-Hilfsmobil.",[27,28,29,30,31,32,33,34],"Homeless Persons","Wound and Injuries","Chronic Wound Care","Homelessness","Environmental Health","Environmental Factors","Psychiatric Disorders","Drug Abuse/Dependence",[36,37,38,39,40,41],"wound care","environmental health","wound and injuries","homeless persons","psychiatric disorders","drug abuse/dependence","NOT_YET_RECRUITING","2026-06-05
2026-06-11","ACTUAL
2026-06-01
2027-12-31
University Hospital Augsburg","OTHER",1
"100636175","using-woundpilot-to-support-wound-care-decisions-in-patients-with-chronic-wounds-100636175","NCT07562269","Using WoundPilot to Support Wound Care Decisions in Patients With Chronic Wounds","Evaluation of the Effect of WoundPilot on the Accuracy and Consistency of Clinical Decision-Making in Chronic Wound Care: A Randomized Parallel Vignette Study Among Primary Care Nurses","Inclusion Criteria:

* Primary care nurses involved in wound care decision-making
* Nurses working in home care or nursing home settings
* Nurses who assess and/or adapt wound management in clinical practice
* Nurses with a qualification corresponding to European Qualification Framework (EQF) level 5 or 6
* Willing and able to provide informed consent

Exclusion Criteria:

* Nurses not involved in wound care decision-making
* Nurses not working in a primary care setting
* Inability to understand the study procedures or complete the assessment tasks
* Refusal or inability to provide informed consent",true
40,[24],"This study evaluates whether a digital decision support tool, called WoundPilot, can help primary care nurses make more accurate and consistent decisions when caring for patients with chronic wounds.

In daily practice, treatment decisions for chronic wounds can vary between clinicians, even when they assess the same patient. This variation may lead to delays in appropriate care, inconsistent treatment choices, or unnecessary referrals. WoundPilot was developed to guide clinicians through a structured wound assessment and link this assessment to clear treatment recommendations.

In this study, primary care nurses will assess a series of clinical cases either with or without the support of WoundPilot. Their decisions will be compared with an expert reference standard to determine whether the use of WoundPilot improves the accuracy of decisions and reduces differences between nurses.

The results of this study will help determine whether WoundPilot can support more consistent and evidence-based wound care in clinical practice.",[68,69,28],"Wound Complication","Wound Care",[71,36,72,73,74,75,76,77,78,79],"chronic wound","clinical decision-making","decision support system","WoundPilot","Primary Care Nursing","Wound Assessment","decision-making","clinical reasoning","hard-to-heal wound","2026-04-24
2026-05-01
"2026-09
University Ghent
"100629882","comparison-of-pain-reduction-in-painful-chronic-wounds-with-and-without-fat-grafting-100629882","NCT07480447","Comparison of Pain Reduction in Painful Chronic Wounds With and Without Fat Grafting","Comparison of Pain Reduction in Chronic Wounds Treated With and Without Autologous Fat Grafting: A Randomized Controlled Trial","PAINFAT","Inclusion Criteria:

* • 3 months old chronic wound (Venous arterial, traumatic, burn and diabetic) having mean VAS Pain score \\>3 at rest

 * Wound area between 4 cm² to 10 cm²
 * Aged 20-60 years.
 * Both male and female

Exclusion Criteria:

* • Active wound infection.

 * Wounds due to malignancy or irradiation.
 * Pregnant or lactating females
 * Smokers","20 Years","60 Years
62,[24],"Chronic wounds, such as those caused by diabetes, burns, or poor blood circulation, often heal slowly and can cause significant pain, affecting daily life and quality of life. This study aims to evaluate whether autologous fat grafting-a procedure in which a patient's own fat tissue is injected into the wound-can reduce pain and improve healing in chronic wounds compared to standard care without fat grafting.

We will enroll 62 adult patients with chronic wounds lasting more than 3 months. Participants will be randomly assigned to two groups: one group will receive fat grafting along with standard wound care, and the other group will receive standard wound care alone. Pain will be measured using a Visual Analog Scale (VAS) over 21 days after treatment.
