Team-Based Shared Decision-Making Program in Cardiovascular-Kidney-Metabolic Health

The investigators are proposing a new team-based shared decision-making (SDM) program. The goal of this study is to see whether this program is practical and whether it may help adults with cardiometabolic risk factors and cardiovascular-kidney-metabolic syndrome. The investigators will enroll adults from a primary care clinic in Maryland. People in the intervention group will take part in the 6-month program in addition to usual care. People in the control group will receive usual care only.

Implementation and Evaluation of a Pharmacist-led Diabetes Care Pathway in Alberta Community Pharmacies

As of 2024, nine percent of Albertans are living with Type 2 diabetes, which increases their risk for cardiovascular disease, stroke, blindness, and kidney failure. Unfortunately, less than half of patients have controlled Type 2 diabetes. We are well aware of the factors which lead to worsening diabetes, but need to give people more support to help them manage their diabetes. Pharmacists are respected health care professionals who are often easier to see that doctors and can help people with diabetes to stay as healthy as possible. This research project aims to see whether a pharmacist service can help improve diabetes management in people with type 2 diabetes compared to usual care from their family physician or nurse practitionner. The potential impact of this project is to empower people with type 2 diabetes to understand their condition, it's management, and to achieve target blood sugar levels, which will ultimately reduce the risk of diabetes-related complications.

Developing an AI Pharmacy Chatbot for the Population of Hong Kong

The primary objective of this study is to is to develop a conversational AI service (chatbot) in Hong Kong to assist patients and caregivers with inquires related to Hospital Authority (HA)-prescribed medications, and to evaluate its effectiveness in answering medication-related questions. The main questions it aims to answer are: 1. How satisfied are the patients and caregivers with this pharmacy chatbot? 2. How will the pharmacy chatbot impact eligible patients or caregivers on their (or the patient they are caring for) medication adherence, knowledge, and the consultation time with HA pharmacists? There will be an intervention group and a control group: 1. The intervention group will be invited to use the AI Pharmacy Chatbot online for 7 days through WhatsApp, a commonly used social media platform in Hong Kong. They can inquire about their medications prescribed under HA and get instant, validated answers from the chatbot. 2. The control group will not use the chatbot during the intervention period. To evaluate the chatbot's usability, researchers will measure patient satisfaction through usability questionnaires issued to the intervention group after their intervention period. Differences in medication adherence, medication knowledge, and HA pharmacist consultation time will also be measured between the intervention and control groups after the intervention period, to determine the chatbot's impact.

Supporting Patients Starting New Medicines: Evaluating the myCare Start Service in Switzerland

In Phase B, the aim is to evaluate the myCare Start service in routine care within the ambulatory primary care medicine and community pharmacy setting in Switzerland. A Type II hybrid effectiveness-implementation study will be conducted to evaluate the effectiveness (improvement in adherence), cost-effectiveness and implementation of the service. This evaluation will allow us to build the necessary contextual relevant evidence base to support sustainable funding of the service in the long term.

Effect of Sinbiotic With Multispecies Probiotics on Liver Parameters Liver Enzymes, Ultrasound, Elastography and Adipokines in Same Cases) in Patients With Metabolic Associated Steatotic Liver Disease.

Scientific hypothesis: the use of a synbiotic preparation with a multi-strain probiotic in patients with MASLD can lead to a decrease in non-invasive elastographic parameters of hepatic steatosis and fibrosis and an improvement in liver function. The main objective of this study is to examine whether the test product affects the improvement of liver function measured by elastographic parameters or at least the prevention of further disease progression. The goal of this clinical trial is to learn if sinbiotics works to improve liver function in adult patients with MASLD The main questions it aims to answer are: Does sinbiotic lowers elastographic parameters od steatosis and fibrosis? Does it change liver function by lowering liver enzymes, blood lipids and sugar? Can sinbiotics lower CV risks and improve quality of life? Researchers will compare sinbiotic to a placebo (a look-alike substance that contains no drug) to see if sinbiotic works in MASLD patients. Participants will: Take sinbiotic or a placebo every day (td) for 9 months Visit the clinic once every 3 months for checkups, and at the begining and after 9 months for blood tests and US with elastography Keep a diary of their symptoms, diet, activity
