Immersive Virtual Reality Versus Conventional Physiotherapy for Shoulder Tendinopathy

Immersive virtual reality (VR) has emerged as a promising tool to support therapeutic exercise by increasing patient motivation, promoting movement through gamification, and potentially reducing pain perception during movement. However, evidence regarding its use in shoulder tendinopathy remains limited. This randomized controlled pilot study will evaluate the clinical efficacy and feasibility of an immersive VR-based exercise program compared with conventional physiotherapy in patients with shoulder tendinopathy. A total of 20 patients referred to the Rehabilitation Department of Hospital Universitario de La Línea de la Concepción will be randomly allocated to either a conventional therapeutic exercise group or an immersive VR-based exercise group. Both interventions will last 4 weeks, with 3 sessions per week, for a total of 12 sessions. The primary outcome will be shoulder pain and disability measured with the Shoulder Pain and Disability Index (SPADI). Secondary outcomes will include pain intensity, upper limb function, shoulder range of motion, isometric strength, kinesiophobia, and patient satisfaction. Outcomes will be assessed at baseline and immediately after the 4-week intervention

PEMF Therapy in Shoulder Tendinopathy

The goal of this clinical trial is to evaluate whether pulsed electromagnetic field (PEMF) therapy delivered via the Super Inductive System (SIS), integrated into a multimodal rehabilitation program, improves pain, function, pressure pain threshold, muscle strength, psychological status, and quality of life in adults with shoulder tendinopathies. The main questions it aims to answer are: * Does the addition of PEMF (SIS) therapy reduce pain intensity and improve pressure pain threshold compared to standard balneophysiotherapy? * Does PEMF (SIS) therapy improve shoulder function, muscle strength, psychological status, and quality of life? The study will compare a multimodal rehabilitation program supplemented with PEMF (SIS) therapy to standard balneophysiotherapy to determine whether the addition of PEMF (SIS) provides superior clinical outcomes. Participants will: * Undergo a multimodal rehabilitation program including balneophysiotherapy * Be assigned to receive either standard treatment or treatment supplemented with PEMF (SIS) therapy * Be evaluated at baseline (admission), immediately after treatment completion (discharge), and at 4 weeks after treatment using validated clinical instruments assessing pain, function, pressure pain threshold, muscle strength, psychological status, depressive symptoms, and quality of life.

The Effect of an Exercise Program Based on Attentional Focus Shifting on Clinical Outcomes in Rotator Cuff Related Shoulder Pain

The aim of this clinical trial is to investigate the effects of an attention shift-based rehabilitation program on clinical outcomes in individuals with rotator cuff-related shoulder pain (RCRSP). The study will evaluate the effects of an attention shift approach, in which motor tasks targeting regions outside the shoulder are performed concurrently with standard shoulder exercises, on pain, functional status, and pain catastrophizing. These results will be compared with those of a standard shoulder rehabilitation program. The main questions this study aims to answer are: Is there a difference in pain levels between individuals with RCRSP who participate in an attention shift-based rehabilitation program and those who participate in a standard exercise program? Is there a difference between these two rehabilitation approaches in terms of shoulder function and pain catastrophizing? Participants will: Be randomly assigned to either an attention shift-based rehabilitation group or a standard shoulder rehabilitation group Participate in supervised exercise sessions twice per week for 6 weeks Attend sessions lasting approximately 60-75 minutes Undergo clinical assessments at baseline and at week 6

Electroacupuncture and Cupping for Shoulder Tendinitis

This study aims to evaluate the effectiveness of electroacupuncture combined with vacuum cupping therapy in treating simple periarthritis of the shoulder associated with the traditional medicine syndrome of Qi stagnation and blood stasis. Shoulder periarthritis is a common condition that causes pain, stiffness, and limited movement of the shoulder due to injury or inflammation of the muscles, tendons, ligaments, and surrounding soft tissues. It can interfere with daily activities such as dressing, lifting objects, sleeping, and working. Electroacupuncture is commonly used to relieve shoulder pain and improve shoulder function. Vacuum cupping therapy is another non-drug treatment that may help improve blood circulation, reduce muscle tightness, decrease pain, and improve mobility. However, there is currently limited evidence about whether combining electroacupuncture with vacuum cupping provides better results than electroacupuncture alone in patients with shoulder periarthritis and Qi stagnation-blood stasis syndrome. This study is being conducted to determine whether the combined treatment is more effective and safe. A total of 54 participants with simple shoulder periarthritis and Qi stagnation-blood stasis syndrome will be enrolled in the study. Participants will be randomly assigned to one of two groups: Group 1: treatment with electroacupuncture alone. Group 2: treatment with electroacupuncture combined with vacuum cupping therapy. All participants will also receive advice on shoulder exercises, posture, and daily activities to support recovery. Treatments will be performed by licensed traditional medicine practitioners. Each participant will take part in the study for two weeks. Assessments will be performed before treatment, after one week, and after two weeks. The study will evaluate: Pain intensity using the Visual Analogue Scale (VAS). Shoulder pain and disability using the Shoulder Pain and Disability Index (SPADI). Shoulder range of motion using the McGill-McRomi Range of Motion Index. Any side effects or unwanted events related to treatment. Possible benefits of participating in this study include reduced shoulder pain, improved movement and daily function, and close monitoring by health care professionals. Participants may also benefit from receiving non-drug therapies at no cost during the study period. The findings of this research may help improve future treatment options for patients with shoulder pain. Possible risks of electroacupuncture include mild pain at the needle site, minor bleeding, bruising, dizziness, or fainting. Possible risks of vacuum cupping include temporary discomfort, skin redness, bruising, or mild pain in the treated area. These side effects are usually mild and temporary. If any unexpected problem occurs, participants will receive prompt medical attention. Participation in this study is completely voluntary. Participants may refuse to join or may withdraw from the study at any time without affecting their usual medical care. All personal information collected during the study will remain confidential and will only be used for research purposes.

Nutrition Impact on PRGF Treatment for Tendinopathy

Given the close correlation reported in the literature between onset and progression of tendon pathology in subjects affected by the alteration of metabolic disorders such as obesity, diabetes mellitus and lipidic metabolic alterations, with the present study the association between the nutritional state, metabolic profile and clinical outcome of patients following treatment with blood-derived orthobiological for shoulder tendinopathies will be evaluated. The primary aim of this study is to identify the proportion of tendinopatic patients responsive and non-responsive to treatment with "Platelet rich growth factors" PRGF at six months follow up and the correlation with their nutritional status.
